I Databases can be linked by computer, e.g. people exposed to a particular product, and people admitted to hospital with a particular problem. Linking a national registry of clozapine recipients to national death records found that clozapine increased the risk of fatal pulmonary embolism and respiratory disorders but reduced the risk of stticide. 

Mortality data are frequently used in cohort studies as opposed to incidence data because of the relative ease of obtaining information on deaths. In particular, the advent of the National Death Index has made mortality data more readily available. While all states maintain registries of incident cancer cases, many of the registries are relatively new and data quality can vary from state to state. Investigators conducting follow-up studies are required to comply with each state s requirements for use of the data. For cancers such as pancreatic cancer where survival is poor, mortality data is an excellent surrogate for the risk of the disease. For other cancers where the survival is much better, such as testicular cancer, mortality is a poor estimator of incidence. Table 15.5 describes the 5-year survival for several selected cancer sites for the period 1996-2004. As evident from the table, there is a considerable difference in survival rates for cancer of different sites. The 5-year survival rate for pancreatic cancer was only 5.1% compared to a survival rate of 98.9% for prostate cancer (SEER 2008). 

The National Death Index (NDl), a service provided by the National Center for Health Statistics, is a central computerized index of death record information on file in the state vital statistics offices. The NDI covers deaths in the United States beginning in 1979. 

Death Valley, California, has historically been a significant source of both colemanite and ulexite, but mining in the Death Valley National Monument has been forbidden as a result of environmental concerns. In 1986, the American Borate Co. ceased mining in Death Valley, but continues to market ore concentrate from inventory as well as borates and concentrates imported from Turkey. 

The Immediately Danger to Life or Health (IDLH) level for CO2, set by the National Institute for Occupational Safety and Health (NIOSH) is 50,000 ppm. At that level, vomiting, dizziness, disorientation, and breathing difficulties occur ifler a 30-minute exposure at a 100,000 ppm, death can occur after a few minutes, even if the oxygen in the atmosphere would otherwise support life.

Ipatieff never received the honor he coveted the most, the Nobel Prize. However, he continued to publish voluminously and, ever the practical scientist, he obtained numerous patents. Ipatieff lived long enough to see the petroleum industry transformed with process technologies that he created and that were rooted in his early scientific research. The first platforming plant, the cuhniiiation of his life s work in catalytic research, came on line just shortly before his death in 1952. 

Red Bull and vodka has been called Ecstasy in a can, a reference to a popular club drug. It has also had its run-in with the law the drink was briefly investigated in 2001 by the Swedish National Food Administration after the deaths of three people who were believed to have drunk Red Bull with liquor. No action was taken against the company, Red Bull GmbH. The drink continues to be available in Sweden and in eighty-eight other countries. 

Increased mortality was observed in both male rats (at doses of 20.4 mg/kg/day and above) and male mice (at doses of 0.46 mg/kg/day and above) in a 2-year bioassay conducted by the National Cancer Institute (NCI 1978). The authors attributed the excessive mortality in the male rats to treatment-related toxic nephropathy. The high mortality in male mice was possibly due to fighting since no other treatment-related cause for the deaths could be determined. Survival in females of both species was unaffected by endosulfan (NCI 1978). However, survival was significantly decreased in female rats that consumed 5 mg/kg/day for 2 years (FMC 1959b), and in female mice that consumed approximately 2.9 mg technical endosulfan/kg/day for 2 years (Hack et al. 1995 Hoechst 1988b). In these studies, survival in male rats was not affected at 5 mg/kg/day for 2 years (FMC 1959b) and survival in male mice was not affected at 2.51 mg/kg/day for 2 years (Hoechst 1988b). 

For nearly a century, epinephrine (adrenaline) has been the cornerstone of the acute management of anaphylaxis [1-6], a sudden-onset multi-systemic allergic reaction that can cause death. The World Health Organization lists epinephrine as an essential medication for anaphylaxis [7], Where national guidelines are available for the acute management of anaphylaxis, they universally recommend injection of epinephrine as the initial medication of choice [8].
