In the past few years, comparisons between invertebrate and vertebrate systems have led to the conclusion that an astonishing number of regulatory mechanisms in mesoderm development have been evolutionarily conserved. In particular, genes that are homologous to tinman and bagpipe appear to have related roles in heart and visceral muscle development, respectively, in vertebrate embryos (Lyons et al., 1995 Tribioli et al., 1997). The enzyme nitric oxide synthase (NOS), that produces NO and citrullin from arginine, occurs as several isoenzymes (Knowles et al., 1989). Type I NOS is a constitutive, calcium and calmodulin-dependent enzyme, present in neurons and, possibly, in glia. Type II NOS is calcium-independent and can be induced in macrophages and glial cells by exposure to bacterial lipopolysaccharide (Galea et al., 1992 Murphy et al., 1993). Type III NOS is the endothelial iso-enzyme. Lyon Diet Heart Study, achieved >70% reduction in CAD events (5), evidence for the existence of other major targets that should be addressed. This short review examines recent paradigm shifts and advances in atherosclerosis research that are likely to influence the future of dietary intervention aimed at preventing CAD. It will consider in turn the shift of emphasis toward unstable plaques, inflammation, oxidative processes, and endothelial dysfunction, as well as the growing interest in gene-diet interactions.
