The first step in treatment of NSCLC involves confirmation of the clinical stage and determination of resectability of the tumor. This decision always should be made by a thoracic surgeon who routinely performs lung cancer surgery. Treatment options depend on the advancement of disease (i.e., local, locally advanced, or metastatic). [Pg.1332]

Biomarkers of Disease. No biomarkers are known that are specific for BCME-induced lung injury. Standard chemical examination of nose and throat can provide an index of local irritation, and examination of sputum for abnormal cell types can provide information on the state of the respiratory epithelium. However, these tests cannot distinguish BCME-induced effects from effects caused by smoking or exposure to other chemicals, and can only discover changes after damage to the tissue has already occurred. Cancer is a group of more than 100 different diseases, characterized by uncontrolled cellular growth, local tissue invasion, and distant metastases. It is second only to cardiovascular disease as a cause of mortality in Americans. More than 1.3 million cases of cancer are diagnosed annually, and cancer claims an estimated 570,280 lives in the United States each year. Pemetrexed is a folic-acid antagonist that disrupts folate-dependent metabolic processes essential for cell replication. It is indicated in combination with cisplatin for the treatment of malignant pleural mesothelioma in patients whose disease is unresectable or who are otherwise not candidates for curative surgery and as a single agent for the treatment of locally advanced or metastatic non-small-cell lung cancer after prior chemotherapy. [Pg.552]

The evidence of a time factor of concurrent chemo-radiation remains a controversial issue. A randomized clinical trial in patients with limited disease small cell lung cancer (SCLC) treated with chemoradiation demonstrated a significantly higher local tumour control when OTT was reduced from 33 to 19 days (Turrisi etal. 1999). A meta-analysis of four randomized clinical trials in patients with Umited disease SCLC revealed that OTT is the most important predictive factor for outcome after chemoradiation (De Ruysscher etal. 2006). Delivery of sophisticated local treatment can only yield a survival advantage for patients with truly localized disease, or those with controlled or controllable systemic disease. The success of stereotactic radiotherapy, e.g., for early-stage non-small cell lung cancer (NSCLC) is critically dependent on the correct identification of node-negative patients. For this purpose, a hybrid PET-CT scanner and the tracer FDG might be used. Correct staging is also required for other diseases commonly treated with high-dose radiotherapy such as prostate cancer.
