COPD exacerbations. Therefore, in exacerbation treatment with antibiotics is justified when the patient has at least two of three features of increased dyspnea, increased sputum volume, and sputum pu-rulence. Antibiotic choice will depend on local experience derived from local bacteriological sensitivity data. Older, less costly compounds such as tetracycline, doxycycline, amoxicillin, erythromycin, cefaclor etc. are often as effective as newer, more expensive ones. If resistant organisms are suspected or when the severity of the patients clinical condition puts them at high-risk of treatment failure, a second or third generation cephalosporin, fluoroquinolone, newer macrolide or broad-spectrum penicillin may be preferred. In cases of recurrent infection prolonged courses of antibiotics continuous or intermittent, may be useful. 

Applications. The kinds of motors that are being used successfully with particular kinds of. chemical process equipment are identified in Table 4.1. As many as five kinds of AC motors are shown in some instances. The choice may be influenced by economic considerations or local experience or personal preference. In this area, the process engineer is well advised to enlist help from electrical experts. A checklist of basic data that a supplier of a motor must know is in Table 4.2. The local equivalence that we have just proved implies that the predictive contents of the Maxwell s theory and of this topological model are exactly the same when referred to local experiments, as most of them are. Accordingly, it is not possible to discern between the two by viewing locally. This is the operative meaning of local equivalence.

Step 4 for those involved in the process is to discuss and decide on the measures of quality pertinent to the drug being evaluated. These become the criteria for drug use. They are developed by the clinicians using the medications and are often based on both evidence from the literature as well as local experience. Often, surrogate markers of disease or outcome, or both, must be used if more clinically relevant data are not readily available in computerized records or in the patient s actual chart.

From the mere fact that CF, can be released from the membrane by EDTA treatment and the enzyme stays in solution without detergents, it is apparent that the catalytic sector has minimal, if any, direct interaction with the lipids of the chloroplast membrane. It is a globular protein that is held to the surface of the membrane via interaction with the membrane sector. Recently it was shown that the y subunit is in immediate contact with the membrane sector and the 8 and e subunits may induce proper binding for catalysis [17,18], The enzyme contains a few well-defined sites that were used for localization experiments by the method of fluorescent energy transfer [19,56-61], These studies revealed the position of those sites and helped to localize the various subunits of CF, in space relative to the chloroplast membranes (for a model of CF, see Refs. 61 and 62). These experiments are awaiting analysis of the amino acid sequence of the y subunit that is now under investigation in Herrmann s laboratory [148], Definite structural analysis could be obtained only after good crystals of the enzyme become available. 

The published tables of animal nutrient requirements and feedstuffs composition are crude approximations across the industry at best. A formulator usually will be more effective if he or she applies local experience in feeding animals of similar genetic origin under the environmental conditions at hand and relies on actual analyses of the ingredients received or the historical performance of the specific supplier—if such information is available. 

Faced with the practical requirement to estimate fault sealing capacity, it is essential to combine an interpretation of the geologic processes relevant to seal formation and evolution with local experience. Any deterministic, process-based analysis will typically be constrained by the limitations of our understanding of the process and/or the availability of quality data. Any dependable analysis based primarily on experience typically requires such a large body of data that the exploration/production decisions of greatest value will have already been taken. It is therefore appropriate to combine the two approaches. However, the more we can reduce our dependence upon empirical or semi-empirical use of local experience, the more commercially valuable will be fault seal analyses. 

The above examples of copper deposition and dissolution represent local experiments. They address the deposition on one section of the substrate and the dissolution of a specific distribution of copper clusters. These local processes can be related to the overall process, since information on the latter is available from the Faradaic current measured in the course of an experiment. Significant discrepancies between local and average kinetics are often but not always observed [13,14]. This reflects the stochastic natiue of electrochemical kinetics on the mesoscopic scale, but there are also systematic deviations, which indicate that the local kinetics may be influenced by the tunneling tip. The elucidation of this influence, which has up to now only been addressed qualitatively, is crucial for the understanding of electrochemical processes on the mesoscopic scale. 

The choice of sampling site depends on the volumes of blood required for analysis and whether the sampling time is during or at the end of a study, when larger samples can usually be taken. The sites for blood collection used for different species vary, and sites are sometimes preferentially chosen on the basis of local experience with a technique, in the absence of data suggesting that one site is best. 

The paraffin TMA provides a tool for high-throughput evaluation of localization initiatives. A large number of samples can be evaluated on a very limited number of slides. The data generated from the localization experiments can be linked to other databases with clinical information. This process provides a mechanism for rapid evaluation of a large number of cases. The data generated are also amenable to statistical evaluation and manipulation. 

Bacterial superinfection is commonly noted 3 to 5 days postexposure. Early, aggressive antibiotic therapy should be directed as specifically as possible against identified organisms. Careful, frequent Gram s stains and cultures of sputum are used to identify a predominant organism. Persistent fever, infiltrates, or elevated white blood cell count in the presence of thickened, colored secretions should prompt the institution of a broad-spectrum antibiotic (such as ampicillin or a cephalosporin). The choice of antibiotic should be based on local experience with either community-acquired or nosocomial organisms. Antibiotics are not used prophy-lactically in this setting such therapy would only serve to select a resistant bacterial population in the injured individual.
