After this initial report, results of the multicenter experience were released in 2002 [22]. From October 1995 to December 1999, 1,684 patients (2,561 pacing and defibrillator leads) were treated with laser sheaths at 89 sites in the USA. Complete removal was achieved in 90% of leads, 3% were partially removed, and 7% were failures. The large number of procedures and increasing number of centers using the procedure reduced the difference in success rate in comparison with mechanical dilatation. Major perioperative complications (tamponade, hemothorax, pulmonary embolism, lead migration, death) were observed in 1.9% of patients with in-hospital death in 13 (0.8%). Minor 

Similar results were shown by the European multicenter experience [23]. Kennergren et al. published the final report of the Pacing Lead Extraction Surveillance Study in Europe (PLESSE) trial in 2007 [24]. From August 1996 to March 2001, 383 leads (170 atrial, 213 ventricular mean implant time 74 months) implanted in 292 patients (mean age 61.6 years range 13-96) were extracted at 14 European centers. Complete extraction was achieved in 90.9% of leads and partial extraction in 3.4% extraction failed in 5.7%. Median extraction time was 15 min. Total complication rate, including five minor complications (1.7%), was 5.1%. Cardiac tamponade occurred in ten cases, but no in-hospital mortality occurred. 

The above-mentioned results demonstrate the high effectiveness of the laser-assisted technique. Although no clearly significant improvement in success rate was observed compared with mechanical dilatation, procedural time was reduced. Concern was raised by the slightly higher incidence of major complications, mainly due to venous or myocardial wall injury, but mortality was very low. The latest reports suggest that highly experienced centers and operators increase the safety of laser-assisted lead extraction.
