Vitamin B complex is the collective term for a number of water-soluble vitamins found particularly in dairy products, cereals and liver.Vitamin B (thiamine) is used by mouth for dietary supplement purposes and by injection in emergency treatment of Wernicke-Korsakoff syndrome. Vitamin B2 (riboflavin) is a constituent of the coenzyme FAD (flavine adenine dinucleotide) and FMN (flavine mononucleotide) and is therefore important in cellular respiration. Vitamin Be (pyridoxine) is a coenzyme for decarboxylases and transamination, and is concerned with many metabolic processes. Overdose causes peripheral neuropathy. It may be used medically for vomiting and radiation sickness and for premenstrual tension. Pyridoxine has a negative interaction with the therapeutic use of levodopa in parkinsonism by enhancing levodopa decarboxylation to dopamine in the periphery, which does not then reach the brain. Following the observation that levodopa-induced dyskinesias were reduced by isoniazid in one patient, a further study was undertaken in 20 others taking levodopa plus a dopa-decarboxylase inhibitor. It was found that isoniazid (average dose 290 mg daily, range 100 to 800 mg daily) reduced the dyskinesias of 18 of the 20 patients. Not understood. Metabolic studies in one patient suggest that isoniazid inhibits dopa-decarboxylase, although other mechanisms have been proposed. The isolated case of hypertension and tachycardia is also not understood, but it has been suggested that it may have been due to a weak monoamine oxidase inhibitory effect of the isoniazid metabolites. Spectrophotometric methods for the determination of catecholamine derivatives pyrocatechol, dopamine, levodopa, and methyldopa have been developed and applied to injections and tablets.
