Q6 Pilocarpine eyedrops are suitable. In severe conditions, in addition to the eyedrops, intravenous acetazolamide and intravenous hypertonic mannitol (an osmotic agent) may be used to reduce pressure. Acetazolamide prevents the actions of carbonic anhydrase in the ciliary body and inhibits bicarbonate synthesis. This causes reduction in sodium transport and aqueous humour formation since there is a link between bicarbonate and sodium transport. [Pg.290]

Human carbonic anhydrase II (HCAII) is present in the nonpigmented epithelial cells of the eye, where it catalyzes the reversible, zinc-assisted hydration of carbon dioxide to generate bicarbonate. Overproduction of bicarbonate results in increased intraocular pressure, which ultimately can produce optic nerve damage and blindness. Accordingly, HCAII inhibitors are used to treat diseases associated with high intraocular pressure, such as glaucoma, because they inhibit bicarbonate production and thereby decrease sodium and fluid secretion into the eye. [Pg.258]

An important gastric secretion is the hydrochloric acid that performs a number of functions in the stomach. This stomach acid is neutralized by pancreatic bicarbonate ion in the duodenum. Excess acid in the chyme stimulates chemoreceptors in the duodenum. This receptor stimulation elicits reflex inhibition of gastric motility. Excess acid also causes the release of secretin and gastric inhibitory peptide from the duodenum.
