Ichthyosis treatment

Treatment of ichthyosis vulgaris and xerosis PO Apply sparingly and rub into area thoroughly ql2h. 

Propylene glycol is an effective keratolytic agent for the removal of hyperkeratotic debris. It is also used in combination with salicylic acid in the treatment of ichthyosis, psoriasis, keratosis pilaris and hypertrophic lichen planus. 

Propylene glycol is used under polyethylene occlusion or with 6% salicylic acid for the treatment of ichthyosis, palmar and plantar keratodermas, psoriasis, pityriasis rubra pilaris, keratosis pilaris, and hypertrophic lichen planus.

The first recorded use of lactic acid was in 1943 by Stern who used it for the treatment of ichthyosis,100 and in the early 1970s and 1980s Middleton101 and Van Scott and Yu102 103 demonstrated the efficacy of these short chain AHAs in ameliorating dry skin in moisturization efficacy studies. 

The efficacy of urea in the treatment of ichthyosis has been investigated in several clinical studies. Different types of evidence exist for the clinical efficacy and the data support at least evidence-level lb...

Table 19.2). In seven patients with severe ichthyosis a pronounced keratolytic effect was noticed and the skin became soft and pliable after treatment with high concentrations (about 10%).17 After treatment with 10% urea (Calmuril ) the number of stratum corneum cell layers was reduced in 6 of 11 patients with ichthyosis.28... 

Evidence from at least one randomized controlled trial (level I) show that 10% urea is effective for the treatment of psorisasis, ichthyosis, and dry feet, and 4 to 10% for the treatment of dry atopic skin and senescent skin. Evidence from another well-designed clinical study (level II) supports the treatment of hand dermatitis with urea. Evidence at level I also exist showing barrier improving effects of urea in both normal and in dry skin disorders (atopic skin, ichthyosis). Furthermore, strong evidence exists (level I) for reduced susceptibility to SLS, but not to other external agents. No evidence has been found for successful treatment of seborrhoic dermatitis, perioral dermatitis, and keratosis pilaris with urea.

In 1990, etretinate (Tigason) was replaced by acitretin (Neo-Tigason), an aromatic retinoid, a carboxylic acid metabolite of etretinate (15). It is effective in pustular psoriasis and psoriatic palmoplantar keratoderma and in combination with PUVA or topical therapy (calci-potriol or glucocorticoids) in the treatment of other forms of psoriasis. It has also been used to treat disorders of keratinization (ichthyosis, palmoplantar keratoderma, Darier s disease) and severe cutaneous forms of lichen planus. It prevents new skin carcinomas in patients with xeroderma pigmentosum and those who are immunosuppressed. The main advantage of acitretin is its short half-life of 50 hours, compared with over 80 days for etretinate (16).
