Evidence from at least one randomized controlled trial (level I) show that 10% urea is effective for the treatment of psorisasis, ichthyosis, and dry feet, and 4 to 10% for the treatment of dry atopic skin and senescent skin. Evidence from another well-designed clinical study (level II) supports the treatment of hand dermatitis with urea. Evidence at level I also exist showing barrier improving effects of urea in both normal and in dry skin disorders (atopic skin, ichthyosis). Furthermore, strong evidence exists (level I) for reduced susceptibility to SLS, but not to other external agents. A 41-year-old man developed suspected occupational hand dermatitis, but had clinical features that were not typical of contact dermatitis, with discrete excoriated papules and some hchenilied plaques on the dorsum of the hands and wrists the palms were unaffected [50 ]. Punch biopsies from the backs of the hands suggested Uchen simplex. However, patch testing was positive with various salts of cobalt. Work-related dermatoses, in particular hand dermatitis, are still among the most prevalent occupational diseases. On a visit to any manufacturing plant, one is likely to encounter workers with hand dermatitis. There is a vast literature on work-related dermatoses, particularly case reports and investigative clinical studies their epidemiology, however, has received little attention. [Pg.3]

In an epidemiological study on the prevalence of HE in different occupational groups, it was noted that in population-based studies the symptoms of contact dermatitis are relatively mild in the majority of cases, judged from the proportion of cases that resulted in sick leave or medical attention (Smit et al. 1993). Of all persons with work-related hand dermatitis, only between 15% and 36% had consulted a physician for treatment, and sick leave due to their symptoms resulted in only 4-9% (Table 3). [Pg.5]

Smit et al. (1994) followed 74 apprentice hairdressers and 111 apprentice nurses from the start of first occupational exposure until the end of their apprenticeship. The average IR of hand dermatitis was 32.8 cases/100 person-years in hairdressers and 14.5 cases/ 100 person-years in nurses. The relative risk of having a dry versus normal skin type was 7.3 in hairdressers and 1.7 in nurses. Apprentice nurses with a history of (atopic) mucosal symptoms had a 3.4-fold increased... [Pg.11]
