The formal safety review can be used almost immediately, is relatively easy to apply, and is known to provide good results. However, the committee participants must be experienced in identifying safety problems. For less experienced committees, a more formal HAZOP study may be more effective in identifying the hazards.

The plant Safety Analysis Report, updated in 1996, the operational documentation and the operational feedback database were used as background documents for the review. In addition, the results of a PSA Level 1 study developed for full power, low power and shutdown operational regimes were available for the review, as well as specific analyses of internal flooding and fires. Plant design documentation was also used to an appropriate extent. However, it should be emphasized that due to the hmited scope and time available for the review, the objectives of the screening were more related to verification of the applicabihty of the approach than to evaluation of the safety status of the plant. Therefore, this section cannot be considered as a formal safety review of the plant. 

The safety review technique is also usehil for small laboratory operations and small changes in existing processes. In these cases, the committee often consists of two or three people and any changes are often less formally recommended.

Another method that is commonly used to identify safety problems in laboratory and process areas and to develop solutions is the safety review. There are two types of safety reviews the informal and the formal. 

Pre-start-up Safety Review (PSSR) approval of the SIS establishes the point where the SIS design and construction is considered complete. All documentation should be formally updated to as-built status, incorporating any modifications made since the last formal drawing or document revision. Once the PSSR has approved the SIS for process unit start-up, formal management of change procedures should be followed to address proposed modification to the SIS or its associated documentation. Any deviation from the approved design basis should be reviewed and approved by appropriate parties prior to change implementation. 

Restart criteria recommendations should receive the same degree of scrutiny as the final and formal recommendations. The manage-ment-of-change and pre-startup safety review implications of each restart recommendation should he considered and evaluated for possible adverse consequences. Simultaneously most petroleum and chemical companies have also brainstormed a safety review which asks "What-If" questions of the process (e.g. SOHIO ca. 1967). This is common practice in the industry and during design phases of a facility but was usually verbal and less formal in its application. Therefore not as much historical documentation is available on it, as compared to the HAZOP method.

A safety review is recommended before any type of combustion experiment, no matter how routine. The extent of the review will depend on how common the test is and the potential for damage or injury. For tests that are commonly done with relatively little chance for damage or injury, a short informal review may be all that is required. For less common tests with a high potential for damage or injury, a more extensive formal review is recommended that could take a significant amount of time to complete. Some type of checklist should be used to ensure that all appropriate factors have been considered in the review. 

Formal reviews normally require appropriate documentation. For example. Excel spreadsheets or Word templates are available for conducting safety reviews to ensure that a specific process has been used in the analysis and to document the results of the review. The documentation should include the team members, the dates and times of the review, the functional area of each team member, the purpose of the review, the equipment and processes being analyzed, key specifications for the equipment (e.g., maximum flow rate), what codes and standards apply to the review if any, and a discussion of past incidents that may be relevant to what is being reviewed.
