Examples of Quality Audit report forms are shown in Tables 9.5 and 9.6 at the end of this chapter. Table 9.5 shows Report Form 1 which includes a record of what has been examined, so that subsequent audits can examine other aspects of the laboratory s operations. Note that when improvement actions are required, the form records not only what needs to be done, but also by when it must be completed, as shown in Table 9.6 (Report Form 2). The form is signed by the auditor and the responsible person from the area being audited. In the UK, the accreditation body UKAS refers to the corrective action as improvement action . This further emphasizes the continual improvement aspect of the ISO/IEC 17025 and ISO 9001 Standards. The examples shown in Tables 9.5 and 9.6 will probably have to be modified to meet a laboratory s particular requirements. It should also be remembered that both report forms should indicate (usually as a footer or header) the title of the document, its issue date, issue number, who authorized the document, page number and total number of pages.

Publish only one official version of the report. Sometimes the team may release a preliminary draff copy that differs from the final version. This can sometimes cause urmecessary and avoidable confusion if the interim report is not handled carefully. If a preliminary draft is published, the team should ensure that all copies of the preliminary version are replaced with the final edition. The preliminary report should be conspicuously identified as a draft. Consider printing a message on each page s footer or header such as Draft— not a final report—subject to change. 

When each new draft is produced, whether it is in development or considered final, each page must have footers indicating the version and the date of preparation. This should prevent confusion about which draft is being reviewed or used and allow easy identification of any pages which... 

Each page of the CRF should include investigator identification, study subject number, protocol number, subject initials, and visit and/or study day. All pages are numbered as v of y pages with the version and date of CRFs in a footer. Each appropriate section requires the signature from the investigator and a final sign-off. 

Images Charts - May be used in visual presentations as long as either the identifying footer of the image remains intact or the photographer/author and Erowid.org are credited visually. If credits for the presentation are also being included elsewhere (for instance at the end of the presentation, or in print), Erowid should be included. 

If you decide to create a resume that contains multiple pages, access the View pull-down menu and select the Header and Footer options. This will allow you to customize the information to appear in the top or bottom margins on subsequent resume pages. 

The exterior walls, the basin divider partitions, and the heat-exchanger flume are constructed of concrete block on poured concrete footers. The basin was made watertight by placing prefabricated asphalt matting directly on the soil and on the inside surfaces of the walls of the still. Two-inch-thick Foamglass insulation was placed around the perimeter walls so that the edge heat losses would be comparable to those of much larger stills. 

Experience gained during construction pointed to several ways in which the still could be substantially improved from the standpoints of sounder construction and lower cost. For example, considerable labor and materials could be saved by eliminating the footers beneath the perimeter walls and supporting the walls directly on the waterproof basin liner. The weight of the superstructure of the still is much less than the bearing capacity of most types of soil, including sand.
