Fazadinium

Gallamine, alcuronium and fazadinium have all been used in the past but are rarely used now in fact only gallamine is still available in some European countries. All of these have significant side effects, particularly on the cardiovascular system. All are long acting and at least gallamine and alcuronium are almost totally dependent on the kidneys for elimination.

It is dose-related (4), the increase in heart rate varying between 30 and 100%, and is associated with a rise in cardiac output and falls in stroke volume and peripheral resistance. Hypertension or hypotension can occur (5,6). If fazadinium is used injudiciously, extreme and dangerous cardiovascular changes can ensue (7). 

Histamine release from fazadinium is very uncommon, but hypotension associated with an urticarial rash and two cases of severe bronchospasm and cardiac arrest (in patients who had also received thiopental) have been reported (8) as probably being due to fazadinium. 

Immunological investigations combined with positive intradermal tests have been used to confirm fazadinium as the causative agent in a severe reaction (9).

Faverin fluvoxamine. fazadinium bromide [ban. inn] is a bisquaternary amine complex heterocyclic compound, which acts as a NICOTINIC cholinoceptor ANTAGONIST, a (competitive) NEUROMUSCULAR BLOCKING AGENT. It can be used as a SKELETAL MUSCLE RELAXANT in anaesthesia, fazarabine [inn, usan] (Ara-Ac NSC 281272) is an analogue of cytarabine, an antimetabolite cytotoxic agent that has been used in ANTICANCER treatment. 

Fazadinium, a competitive blocker not available in the United States, represents a different approach to shortened duration of action by facilitating metabolism. Here, hepatic azoreductase cleavage (Chapter 3) will result in an innocuous metabolite and N2. However, the rate of cleavage in humans is much slower than it is in some animals. 

Fazadinium, similar to anticholinergic properties and raises heart rate and cardiac ontpnt (see also Fignre 79).
