When antibodies to CK7, CK20, CK5, or other keratins are combined in panels with various antibodies to other intermediate filaments (e.g., vimentin), antibodies to supplemental epithelial antigens (e.g., CEA, EMA), or antibodies to specific cell products or transcription factors (e.g., neuroendocrine granules, TTFl, WTl), a more specific identification of cell type may be rendered. An algorithmic approach as shown in Fig. 8.20 may be used in arriving at the correct diagnosis.

The amplification of SCCA mRNA may be a useful method for the detection of micrometastases and the monitoring of squamous cell carcinoma such as squamous cell carcinoma of uterine cervix, head and neck and esophagus. The simultaneous amplification of other epithelial antigens such as CK19 as multiplex PCR is recommended for precise results.

Form protective physical barrier on epithelial surfaces, are involved in cell-cell interactions, and may contain or mask certain surface antigens. 

Resistance to phagocytosis is sometimes associated with specific components of the cell wall and/or with the presence of capsules surrounding the cell wall. Classic examples of these are the M-proteins of the streptococci and the polysaccharide capsules of pneumococci. The acidic polysaccharide K-antigens of Escherichia coli and Sal typhi behave similarly, in that (i) they can mediate attachment to the intestinal epithelial cells, and (ii) they render phagocytosis more difficult. Generally, possession of an extracellular capsule will reduce the likelihood of phagocytosis. 

An alteration in the inflammatory response regulated by intestinal epithelial cells may also contribute to development of IBD.
