I had the privilege of working with that Panel, which some of you may recall as the "Charple Panel," named after Its Chairman, Bob Charple. We argued and debated and studied the problems faced by inventors, innovators, and entrepreneurs. For a year we did this, and then It was my job to synthesize all of that into a final report. The title of it was, as some of you may recall. Technological Innovation Its Environment and Management. 

The "Charple Report" to the Secretary of Commerce in 1967 presented succinct and timely proposals from a blue-ribbon panel of private citizens on "Technological Innovation Its Environment and Management." Many of these proposals were quite similar to those made by the private-sector Task Forces In the current Domestic Policy Review. 

ABSTRACT By analyzing the hierarchy of safety factors in purification plant of natural gas, they are divided into personnel, equipment, environment and management. After the study of safety index, evaluation methods and fuzzy arithmetic method for each hierarchy, its fuzzy evaluation flow is given. During the evaluation, the weight of the factors and each hierarchy is decided by analytical hierarchical process, and the operational criterion adapts maximum membership degree. And this model is exemplified in purification plant of natural gas. The results show that the second fuzzy evaluation is effective to assess purification plant of natural gas. 

It has been universally accepted that, hazard is the cause (root) of accident, that is to say, effective hazard control can prevent accident from occurring. According to GB13861-2009, hazard is categorized into four types human, thing, environment, and management factors. 

Thousands of incidents occur every day in workplaces. The failure of people, equipment, the environment, and management to behave or react as expected causes most of these incidents. Investigating incidents is important because it determines how and why failures in the management system may have occurred [6]. 

Take action to reduce these risks. The guidance includes a number of useful tables giving non-sector specific examples of factors relating to the design of shift work schedules, the physical environment and management issues such as supervision. 

All wastes, whether or not they are considered hazardous under the law, must be evaluated for the potential harm they present to human health and the environment and managed accordingly. 

As a result, compared with Figure 9.3, under the same uncertain environment and managing strategy, the international sales strategy influences the performance. International sales strategy at a high level is better (improves 9%) than at a low level. 

Human Performance Evaluation identifies feetors that influence task performance. The focus of this analysis method is on operability, work environment, and management factors. Man-machine interfece studies...

VII. CAUSAL FACTORS. Describe events and conditions that contributed to the accident. Include information on worker, machinery and equipment, environment and management.
