Emergency priorities

After World War II (WWII), the National Institute of Health broadened its research scope to include chronic diseases such as cancer and mental illness. In 1948, the institute was expanded and the name was changed to the National Institutes of Health (NIH). At present, the NIH consists of 17 institutes and is an agency within the U.S. Public Health Service, Department of Health and Human Services. Its budget in 1999 was U.S. 15.6 billion. The establishment and subsequent growth of the NIH reflects the emerging priority placed on scientific inquiry and treatments based on xmderstanding pathologic processes at a fundamental level. This trend continues currently as evidence-based medical practice.

In this handbook we have sought to present a unique and comprehensive account of the current state of the science in this wide area. Four chapters by the editors set out the underlying principles. Sixteen chapters written by 38 selected experts discuss specific transport processes in a consistent, user-fliendly format. Transport data and correlations are provided along with examples of calculation procedures. The last chapter offers our personal glimpse of future needs and emerging priorities. 

Preventive medicine through vaccination continues to be the most cost-effective pubHc health practice, even with the drastic advance in modern medicine. Mass vaccination programs have eradicated smallpox from the earth. The World Health Organization (WHO) has a major campaign underway to eradicate poHo by the year 2000. The development of vaccines has saved millions of Hves and prevented many more from suffering. However, there are stiU many diseases without effective vaccines, such as malaria. With the recent emergence of antibiotic-resistance strains and exotic vimses, an effective vaccine development program becomes a top priority of pubHc health poHcy.

Apart from of investigating the concentration of priority pollutants to assess the temporal and geographic trends as it has been done in the Ebro river basin until nowadays, the SCARCE project pretends to evaluate the consequences of the climate change in the water quality as well as predicting new environmental risks derived from water scarcity. This will be done by determining the presence of new priority (i.e. perfluorinated compounds) and emerging toxicants in Mediterranean river ecosystems in the Iberian Peninsula. In a subsequent step, the effects of chemical and environmental stressors on the biota will be assessed by combining field and experimental studies. The effects of multiple stressors will be addressed from a multi-biomarker perspective [27]. 

Occurrence and distribution of a wide range of priority and emerging contaminants (pharmaceuticals, drugs of abuse, polar pesticides, etc.)... 

If no conflict free shortest path can be found, the conflict resolution tries to solve the conflict by waiting, speed reduction or evasion of the currently considered AGV. The applied priority rule is a First-Come-First-Serve heuristic. By using a FCFS-priority rule, paths that were once scheduled stay conflict free in the future [6, 7]. If the detected conflict can be solved, the resulting path is again analyzed for conflicts because new conflicts can emerge from the resolution of an earlier conflict. 

From the wide variety of emerging pollutants of industrial origin that could be considered here, bisphenol A (BPA) and phthalate esters (PE) are of especial relevance not only because of the high volumes produced and their widespread use, but also because of their demonstrated toxicity, particularly as endocrine disrupters. Both of them have been included in the final report of the European Commission toward the establishment of a priority list of endocrine disrupter chemicals, EDCs [3], and have been rated as of high risk of exposure for human and wildlife populations. Because of their structural characteristics these compounds cannot be included in any of the groups described above, so they will be described in this section (see Fig. 10). 

All new drug applications are expected to be in the CTD format according to ICH guidelines (see Section 7.11). Priority reviews are applicable for orphan drugs and those drugs for the treatment of serious illnesses. A restricted approval system has been implemented for emergency drugs to prevent spread of diseases. In this case, the standard review procedure is not applicable.
