Emergency instructions requirements

The Licensees are required to produce an Emergency Plan for each NPS and this is approved by the Nuclear Installations Inspectorate (Nil). The Plan is implemented by Emergency Instructions which detail the duties and responsibilities of all those personnel involved with the Emergency Arrangements. The Instructions also list the inventory of all plant and equipment which must be available and ready for use in the event of an emergency situation arising. 

The existing standard at 1926.21(b)(6) states, All employees required to enter into confined or enclosed spaces shall be instructed as to the natvu e of the hazards involved, the necessary precautions to be taken, and in the use of protective and emergency equipment required. The employer shall comply with any specific regulations that apply to work in dangerous or potentially dangerous areas. ... 

Prepare for emergencies that require first aid by taking a first aid course (that includes CPR instruction) and knowing the location of first aid equipment in the lab. 

On Piper Alpha, primary (helicopter) escape was simply not an option because of the lire. Smoke had quickly engulfed the landing pad. Many of the crew mustered in the dining room of the Accommodation Module as the emergency plan required, but no clear further instructions were ever given. No secondary escape was even at-tanpted, and no lifeboats were launched. When the final huge explosion occurred at 2320 hours the Accommodation Module fell into the sea, but by that time the 81 people inside the module had already died from smoke inhalation. 

Emergency instructions provided on notices should contain information relating to such issues as the action to be taken when a fire is detected, the action to be taken when the alarm is heard and any special additional instructions that may be required. 

The design assessments and basis of safety must be recorded and available for consultation, and the process must be operated as required. The operating phase involves two sets of process documentation—the detailed technical documentation, and safe operating instructions (HSE 2000) for every operational phase, including startup, shutdown, maintenance and emergency operations. 

Training of emergency responders (firefighters, police, and emergency medical services) for responding to weapons of mass destruction should emphasize critical concepts for self-preservation and effective casualty management. Training should include both classroom instructions on basic concepts and principles, hands-on demonstrations of required skills, and drills to reinforce basic procedures. For all situations that require the handling of toxic materials, the employees concerned should be provided with an instruction manual, prepared if necessary by a consultant, describing all foreseeable situations, routine and emergency, and the actions that should be taken if they should occur. 

Written instructions in case of an adverse reaction usually require that the patient be directed to an emergency room for evaluation, monitoring, and treatment. Nonspecific treatment of a hypertensive crisis is as follows ... 

The product requirements are reviewed before the contract or tender is accepted. Product requirements are defined and communicated to the personnel responsible for the quality production. The unresolved and ambiguous issues are resolved and documented. The changes made in the product specifications are approved before execution of the order. Provisions are made for removing absolute drawings, procedures, work instructions, and specifications from all work areas to ensure that they are correctly authorized versions. Provisions are made for emergency changes necessary to prevent the production of nonconforming material. 

All personnel working within a facility should always be trained to recognize and respond to an emergency, including instruction in how to notify appropriate plant officials and what actions to take in case an evacuation is required (29 C.F.R. sec. 1910.38), (CCPS, 1995). 

System boot and shutdown This is a special SOP that should contain all the specific instructions for starting up and shutting down the system. This SOP may be required in an emergency and therefore should be written well and be easily available for use. 

Asymptomatic patients who have ingested hydrogen cyanide solutions or those with direct skin or eye contact require observation in the emergency department for at least 4-6 h. Those who remain asymptomatic should be discharged with instructions to return if symptoms develop. The ATSDR Medical Management guidelines include a useful patient information sheet (31). 

Computer-based instruction and interactive video both represent areas where some data reporting advantages have emerged specifically, reductions in the time required to reach threshold performance levels (Fletcher 1990 Kuhk and Kuhk 1987). Results of an analysis of 28 studies, conducted by Fletcher (1990), suggested that interactive video-based instruction increases achievement by an average of 0.50 standard deviations over conventional instruction (lecture, text, on-the-job training, videotape).
