Sodium is contraindicated in patients with hypernatremia, fluid retention, and when the administration of sodium or chloride could be detrimental. Sodium is used cautiously in surgical patients and those with circulatory insufficiency, hypoproteinemia, urinaiy tract obstruction, congestive heart failure, edema, and renal impairment. In normal pregnancy there is hypouricemia during the first months, with an increase to slightly above the normal near term (Resen Steenstrup, 1956). Mull and Bill (1945) likewise described a hypouricemia in pregnancy. Munro Kerr et al. (1948) noted that the levels of uric acid in toxemia of pregnancy are increased in the absence of severe kidney injury. Similar observations were made by Lindeboom (1947) in eclampsia. Mukherjee and Govan (1950) reported an elevated level of uric acid in the tissue fluid in the edema of pregnancy in comparison with the tissue fluid of healthy mothers at term. [Pg.271]

Another condition that can impair venous return is pregnancy. As the uterus enlarges during gestation, it may cause compression of the veins draining the lower extremities. Once again, venous and capillary pressures are increased. Filtration is enhanced, reabsorption is inhibited, and edema develops in the lower extremities. [Pg.224]

Pregnant women should be advised to contact their physician before taking spironolactone, since routine use of diuretics during normal pregnancy is inappropriate and exposes the mother and her fetus to an unnecessary hazard. Diuretics do not prevent development of toxemia of pregnancy, and there is no satisfactory evidence that they are useful in the treatment of toxemia. Diuretics are indicated only in the treatment of edema due to pathologic causes, or as a short course of treatment in patients with severe hypervolemia. [Pg.312]

The thiazolidinedion are contraindicated in patients witii a hypersensitivity to the drug or any component of the drug and severe heart failure. These dni are Pregnancy Category C dru and should not be used during pr nancy unless tiie potential benefit of therapy outweighs the potential risk to the fetus.
