The biodistribution of the ideal radiopharmaceutical would show extremely rapid distribution via the circulatory system to the organs of interest and htfle distribution to others. It would be rapidly extracted by the organ or tissue of interest differentially in a way that reflects the disease process of interest. Ideally, abnormahties should be defined by substantial increases in the concentration of the agent because lesions defined by a decrease from surrounding concentrations, ie, cold-spot imaging, are more difficult to image. 

The use of synthetic organic chemistry to create molecules that will alter in a useful way some disease process in a living system. 

An alternative approach involves testing of new drug entities on whole-cell systems and measuring effects on integrated cellular pathways. Favorable phenotypic responses are identified with this approach that may better produce alteration of multicomponent disease processes.

The primary goals of treatment are correction of the intraabdominal disease processes or injuries that have caused infection and drainage of collections of purulent material (abscess). A secondary objective is to resolve the infection without major organ system complications (e.g., pulmonary, hepatic, cardiovascular, or renal failure) or adverse drug effects. The three kinds of disease process we have surveyed in this chapter are the natural analogues of experimental probes of the brain systems that normally alter brain states in animals. Not surprisingly, they alter consciousness in informative ways in humans. Strokes are the equivalent of electrolytic and excitatory neurotoxic lesions seizures are the equivalent of direct electrical and chemical stimulation alcohol is the equivalent of parenteral drug administration. In reading across the three disease classes in search of unifying concepts, I hope to stimulate a new era of experimental work in animals, as well as to celebrate what we have accomplished in the arena of human neuropsychology.
