Decrement time

SIGN is used to determine whether the carriage position is incrementing or decremented. TIME is the sampling time of the proportional control algorithm. [Pg.553]

It is important to realize that the CSHT does not predict the time to patient awakening but simply the time until the plasma concentration of a drug has fallen by half. The patient may need the plasma concentration to fall by 75% in order to awaken, and the time taken for this or any other percentage fall to occur is known as a decrement time. [Pg.114]

The CSHT is, therefore, a form of decrement time when the specified percentage is 50%. The first role of a reservoir is to impose on the system a gradient that makes the subsystem structure nonzero. The adiabatic flux that consequently develops continually decreases this structure, but the second role of the reservoir is to cancel this decrement by exchange of variables conjugate to the gradient. This does not affect the adiabatic dynamics. Hence provided that the flux is maximal in the above sense, then this procedure ensures that both the structure and the dynamics of the subsystem are steady and unchanging in time. Both THC and ethanol increase reaction time, and produce decrements in standing steadiness and psychomotor coordination (Belgrave et al. 1979). Whereas the peak effects of ethanol appeared quickly and wore off quickly (after 280 minutes), THC s effects were slower in onset and longer in duration. The effects of combined THC and ethanol are additive and not synergistic, and THC did not alter blood-ethanol levels. Usual dose Initial dose of 25 mg twice/day, with increases in increments of 25 to 50 mg 2 or 3 times/day on the second and third day, as tolerated, to a target dose range of 300 to 400 mg/day by the fourth day, given 2 or 3 times/day. Further dosage adjustments should occur at intervals of at least 2 days. Dose increments/decrements of 25 to 50 mg twice/day are recommended. Metabolic acidosis Hyperchloremic, nonanion gap, metabolic acidosis is associated with topiramate treatment. This metabolic acidosis is caused by renal bicarbonate loss because of the inhibitory effect of topiramate on carbonic anhydrase. Generally, topiramate-induced metabolic acidosis occurs early in treatment, although cases can occur at any time during treatment. Bicarbonate decrements usually are mild to moderate rarely, patients can experience severe decrements to values below 10 mEq/L. Conditions or therapies that predispose to acidosis may be additive to the bicarbonate lowering effects of topiramate. If metabolic acidosis develops and persists, consider reducing the dose or discontinuing topiramate. [Pg.1267]
