A 58-year-old man had bleeding gastric varices injected with a 50/50 mixture of enbucrilate and lipiodol, but a follow-up X-ray showed radiopaque material in the left lobe of the liver and a CT scan showed cyanoacrylate embolization via the right gastric vein into the main and left portal veins [32 ]. A CT scan 2 years later showed atrophy of the left-lateral segment of the liver, with portal-vein thrombosis and little residual lipiodol retention. [Pg.1014]

Cyanoacrylate (Histoacryl) is a tissue adhesive used in duraplasty. The sites at which it is used should be carefully chosen. Cyanoacrylates are also in use for embolization of arteriovenous malformations in the brain. The risk of this procedure is the creation of pulmonary emboli after acrylate glue injection, particularly when delivery systems without flow arrest are used in high-flow vascular brain lesions. Techniques using acetic acid to delay polymerization time and sandwich techniques, in which glue is pushed with dextrose, appear to be more likely to cause this complication (1). [Pg.1022]

N-butyl cyanoacrylate is the most commonly used cerebral AVM embolic material, and is considered by some to be the most effective (Finfante and Wakhloo, 2007). While its drawbacks include tissue adhesiveness and optimization of polymerization time, the main advantage of this material is its permanency. The curative capability of n-BCA, seen both after initial embolization... [Pg.189]

In case of high-risk (Borden II-III) DAVMs, particulate embolization as a sole treatment does not provide safe and permanent prevention from subsequent bleeding. If transarterial embolization is considered in such cases because neither surgery nor transvenous embolization is feasible or recommended, liquid embolics should be chosen (Fig. 4.21).
