Gastric bleeding

Possible adverse reactions include cramping diarrhea gastric bleeding ulceration gastritis vomiting drowsiness lethargy headache mental confusion ataxia irregular menses carcinoma of the breast. 

Among the nonselective cyclooxygenase (COX) inhibitors, there is no clear-cut statistical evidence for the superiority of one or another of these useful drugs. Individual patients may do better with some than with others, and there are differences in side effects, primarily gastric bleeding and renal toxicity, which can be especially serious with the prolonged administration of high doses—necessary in chronic diseases such as... 

Adverse effects include nausea, vomiting, anorexia, gastric bleeding, diarrhoea, dizziness, frontal headache, confusion, depression, psychosis, hallucination, leukopenia, epigastric distress and rarely aplastic anaemia.

Although ethanol itself has no effect on bleeding time, it enhances the effect of aspirin when given simultaneously or up to at least 36 hours after aspirin ingestion (99). Ethanol also promotes gastric bleeding. 

Careful monitoring of patients with a history of ulcers is recommended. Gastric bleeding, nau.sca. diarrhea, dizziness and other adverse effects have been noted, but with a lower frequency than with aspirin. Sulindac i.s recommended for RA. OA. and ankyUssing spondylitis in a ISO- to 200-mg dose, twice daily. - It is available as labIcLs (ISO-jmI 200 mg). 

Determine if there is a history of gastric upset, gastric bleeding, or liver disease. 

Indomctacin is one of the more effective agents, but has a higher incidence of adverse effects indiiding ulceration, gastric bleeding, headaches and dizziness, it may also cause blood dyscrasius. 

If the client has black, tarry stools the health-care provider should be notified because this is a sign of gastric bleeding. One aspirin a day should not cause bleeding therefore, this is not the most important information to teach the client. 

A complication of long-term aspirin use is gastric bleeding, which could result in dark, tarry stools. This data would warrant further intervention. 

B. Selective serotonin reuptake inhibitors. Studies have shown that citalopram and serotonin in doses of 20 0 mg/day are effective in the treatment of painful diabetic neuropathy but not as efficacious as tricyclic antidepressants [29], However, they do have less adverse effects, and accordingly they are recommended to be given to patients that do not tolerate tricyclic antidepressants. Upper gastric bleeding is a reported adverse effects and concomitant use of NSAID increases this risk. 

A small increase in the gastrointestinal blood loss caused by aspirin occurs in patients if they drink alcohol, but any increased damage to the lining of the stomach is small and appears usually to be of minimal importance in most healthy individuals. However, heavy drinkers who r ularly take aspirin should be warned of the increased risk of gastric bleeding. Some limited information su ests that aspirin can raise or lower blood alcohol levels. 

Paracetamol is still considered to be safer than aspiring (p.385) or NSAIDs (p.427), as an analgesic in the presence of an anticoagulant because it does not affect platelets or cause gastric bleeding.
