The anatomy of the distal colon, with its thick muscular walls, suggests a predominantly propulsive activity. Studies with single administrations of pellets or Pulsincap devices suggested that the distal part of the transverse colon area is difficult to treat since this area and the descending colon function as a conduit. Steady-state measurements confirm this assertion (48) and Weitschies group have also reported data showing mass movements propel objects quickly through the distal transverse colon. 

The mechanisms of action of many laxatives are not well understood due to the complex factors that affect colonic function. However three general mechanisms can be recognized (1) fluid retention in colonic contents thereby increasing bulk, (2) direct or indirect effects on the colonic mucosa to decrease net absorption of water and NaCl and (3) increase of intestinal motility.

There is a growing interest in the inclusion of soluble fibre into the diet to help improve health. This is extending to drinks as well, with such fibre being added to milk- and fruit-based products such as smoothies . One source of soluble fibre which has attracted attention over the last few years is inulin or oligofiuc-tans. Inulin consists of oligosaccharides that are extracted from chicory or Jerusalem artichokes and that are claimed to improve colon function and to have prebiotic properties, enhancing the working of the gut. Inulin is a complex carbohydrate which can be assayed in a number of different ways. However, there are two published methods in the AO AC manual for its analysis (997.08 and 999.03).

But the most interesting change in Mrs. I. s case was her colon function (which she hadn t mentioned at the beginning of our discussion). She had diarrhea for 13 years without let up. About three months after starting the treatment she passed a rubbey stool which was a foot and a half long She has had perfectly normal bowel movements since that disturbing episode.
