Figure 9 illustrates the stabilizing effect of methylcellulose on the foams generated by a suspension of effervescent antacid granules containing magnesium trisilicate and ammonium hydroxide. Samples 1 and 2 are identical, except for 0.01% of methylcellulose contained in sample 1. The foam in sample 1 (methylcellulose) is stable even after 5 minutes, while the sample without methylcellulose has virtually no foam after 30 seconds. 

Osmotic diarrhea results from the intake of unabsorbable but water-soluble solutes in the intestinal lumen leading to water retention. Common causes include lactose intolerance and ingestion of magnesium-containing antacids.

Other causes include magnesium-containing antacids in patients with renal insufficiency, enteral or parenteral nutrition in patients with multiorgan system failure, magnesium for treatment of eclampsia, lithium therapy, hypothyroidism, and Addison s disease. 

Swallow tablets whole do not chew, crush, or dissolve. Tablets may be taken with meals to minimize Gl effects. Avoid coadministration with magnesium-containing antacids.

Diarrhea Diarrhea (13% to 40%) is dose-related, usually develops early in the course of therapy (after 13 days), and usually is self-limiting (often resolving after 8 days), but requires discontinuation of misoprostol in some of patients. The incidence of diarrhea can be minimized by administering after meals and at bedtime and by avoiding coadministration of misoprostol with magnesium-containing antacids. 

The absorption of tetracyclines is markedly reduced by aluminium and magnesium containing antacids. Tetracyclines may chelate other ions, in particular iron salts, with resultant poor absorption of both drugs. This interaction can be avoided by giving iron salts either 3 hours before or 2 hours after the tetracycline. 

The patient receiving chronicrenal dialysis should not take magnesium-containing antacids during vitamin D therapy...

Antacids apparently do not affect the systemic availability of oral contraceptives, but since magnesium-containing antacids can cause diarrhea, they might reduce absorption of contraceptive steroids. 

The primary antacids can be classified as aluminum-containing, magnesium-containing, calcium carbonate-containing, sodium bicarbonate-containing, or a combination of any of these classifications. These drugs are typically taken orally, either as tablets or as a liquid oral suspension.
