You refer Gail to the psychiatrist, who instructs her to discontinue imipramine and orders a blood level to be drawn that afternoon. The psychiatrist suspects that Gail s metabolism of imipramine may be impaired by the addition of cimetidine. The symptoms that Gail describes are consistent with tricyclic toxicity. Cimetidine is known to inhibit the ability of liver enzymes to metabolize numerous other medications, including antidepressants (American Society of Hospital Pharmacists 1993). [Pg.34]

Initial complaints, which usually diminish with time, include headache, lethargy, and muscle weakness. Nearly half of patients experience a fine hand tremor at some time during lithium treatment. For most people, this initial symptom remits however, for 10 percent of patients, it is a continuing side effect (American Society of Hospital Pharmacists 1993). Often the tremor becomes worse when the individual reaches for something. Sustained tremor, at therapeutic levels, can be managed with reassurance, and by limiting stimulants, such as caffeine, which may exacerbate the tremor. Sometimes this tremor is treated with beta blockers, usually propranolol (In-deral). [Pg.161]

Lithium s benefit in mania may be linked to effects on dopamine and norepinephrine, possibly by preventing dopamine receptor supersensitivity in the manic individual. Additionally, lithium blocks some cocaine- and amphetamine-induced symptoms of mania, which are thought to be mediated by stimulant-related increases in CNS dopamine concentration. The action of lithium on norepinephrine is variable, causing an initial prolonged increase in reuptake, although this effect is not evident with long-term administration (American Society of Hospital Pharmacists 1993). [Pg.164]

Lithium s serotonergic activity is postulated to be responsible for its role in preventing depressive episodes of bipolar disorder. Some bipolar patients demonstrate low CNS concentrations of serotonin. Lithium may contribute to serotonin production by increasing the uptake of the tryptophan, a serotonin precursor (American Society of Hospital Pharmacists 1993). [Pg.164]
