Bishydroxycoumarin (dicoumarol) is a natural occurring anticoagulant found in sweet clover. A number of coumarin derivatives have been synthesized as anticoagulants, warfarin, phenprocoumon and acenocoumarol being most frequently used. The nonpolar carbon substituent at the 3 position required for activity is asymmetrical. The enantiomers differ in both pharmacokinetic and pharmacodynamic properties. The coumarins are marketed as racemic mixtures. 

Phenprocoumon has a long plasma half-life of 5 days and thus a duration of action that can last 7-10 days. On the other hand acenocoumarol has a half-life of 10-24 hours and therefore a shorter duration of action. The half-life of warfarin ranges from 25-60 hours and its the duration of action is 2-5 days. Both warfarin and phenprocoumon are highly protein bound and interactions may occur with other drugs that bind to albumin.

It is most commonly used oral anticoagulants employed for long term anticoagulant therapy and having less toxic effects including alopecia and dermatitis. It is rapidly and completely absorbed from intestines and is 99% plasma proteinbound.lt crosses theplacentalbarrier and is secreted in milk. The other properties are same as acenocoumarol. 

The mechanism of action and adverse effects are same as acenocoumarol. It produces more serious nonhaemorrhagic toxic effects, their use is now very limited. 

G. Other applications NovoSeven may be effective for acute bleeding or hemostasis during surgery. In one study NovoSeven reversed the effects of the oral anticoagulant acenocoumarol on the prothrombin time and International Normalized Ratio (INR) in healthy volunteers, without evidence of systemic coagulation. It may also transiently correct elevated prothrombin time in patients with cirrhosis-induced coagulopathy.

Intravenous methylprednisolone (1 g/day for 3 days) has been reported to inhibit the metabolism of oral anticoagulants (acenocoumarol and fluindione) in 10 patients, increasing the INR by 8 (range 5-20) (478).
