Of 61 patients with ischemic heart disease and dyslipi-demia treated with nicotinic acid 1.5 and 3.0 g/day, 32 patients were withdrawn, 18 because of adverse effects and 14 for reasons not related to nicotinic acid (32). Of the 29 patients who finished the study, adverse effects included dryness of the skin (14%), acanthosis nigricans (10%), fatigue (6.9%), nausea (6.9%), abdominal pain (3.4%), diarrhea (3.4%), and anorexia (3.4%) the figures in parentheses were the incidences at 33 weeks. Flushing occurred more often at 18 weeks than at 33 weeks (24 versus 6.9%), as did pruritus (35 versus 28%), suggesting tolerance to these effects. [Pg.562]

Nicotinic acid disturbs the normal cornification of the skin, occasionally leading to reversible ichthyosis and acanthosis nigricans (SEDA-19, 206). Two mechanisms are advanced to explain this effect insulin resistance induced by nicotinic acid, with compensatory hyperinsu-linemia (which in turn leads to increased insulin binding to insulin-like growth factor receptors and stimulation of keratinocytes), or a disturbance of epidermal lipid homeostasis. [Pg.562]

Skin problems can be persistent in a proportion of patients, variously estimated at 10-59%, and this can severely limit adherence to therapy. The skin reaction can be ameliorated by concomitant use of non-steroidal anti-inflammatory drugs such as aspirin and indometacin (SEDA-15, 412). Transient exanthems, pruritus, and sometimes wheals are seen, as well a uniform dryness and scaling of the epidermis, brown pigmentation, and even on occasion an acanthosis nigricans-like dermatosis (15). Persistent rashes can also occur. Doses in excess of 5 g/day are routinely associated with skin manifestations and can on occasion cause liver damage, gout and ulcer formation. These reactions can be associated with nicotinic acid rather than nicotinamide, which is sometimes recommended as an alternative (37). Increased hair loss has been described. [Pg.562]
