Written warning

In the survey countries, the types of sanction imposed for a violation range from verbal and written warnings, through fines, prohibition and correction of the advertisement and revocation of registration, to imprisonment. The fines charged for violations in Australia are relatively high. Fines for violations in Estonia (US 4 286 to US 7 143) are much higher than those in either Malaysia (US 780 to US 1 300) or Cyprus (US 900).

If an employee does not respond to a verbal warning, a more formal written warning is issued. A written warning is the first formal step in progressive discipline that may result in eventual discharge of the employee. It differs from verbal warnings, which are relatively informal acts that only require the manager to note the time and place of the reprimand and what... 

The written warning should describe the unacceptable behavior clearly, previous warnings, specific expectations of future behavior to be achieved by a precise deadline, and the consequences of not meeting expectations. For example, You were verbally warned about tardiness on January 16 of this year. You have continued to be tardy at a rate above that specified in your performance standards. If you are late for work more than twice within the next month, you will be suspended for one day without pay. As shown by this example, it is essential for a manager to keep good records of previous warnings because they will be used as the basis for potential written warnings. 

Suspensions are punitive actions meant to demonstrate the seriousness of a situation. Sometimes written warnings do not result in improved employee performance and need to be backed up by actions. Suspensions are meant to act as a final warning that current behavior is unacceptable. Like written warnings, they must be crafted carefully to include previous warnings, requirements for future actions, and consequences for not improving behavior (e.g., termination). 

If the results of the literature searches and physical tests detect a hazard that we feel is unacceptable, we will usually try to avoid the hazard by changing process conditions and including written warnings in the procedure. Enforcement actions are designed to respond to non-compliance with specified conditions and requirements. The action shall be commensurate with the seriousness of the non-compliance. Thus there are different enforcement actions, from written warnings to penalties and, ultimately, withdrawal of an authorization. In all cases the operator shall be required to remedy the non-compliance, to perform a thorough investigation in accordance with an agreed time-scale, and to take aU necessary measures to prevent recurrence. Deviations from, or violations of, requirements [of the authorization], or unsatisfactory situations which have minor safety significance, may be identified at facilities or in the conduct of activities. You can also be dismissed if you are often late or your behaviour is inappropriate to the type of work being done. You should have verbal or written warnings before you are dismissed. 

The consequences for drivers found to have an alcohol concentration of 0.02 or greater but less than 0.04. (10) At a minimum, drivers who test with a BAG of between 0.02 and 0.04 are prohibited from performing any safety-sensitive function for a minimum of 24 hours. This is a regulatory requirement. But are there any other consequences, as a matter of company policy, for these drivers Any additional consequence — whether in the form of a written warning, suspension, etc. — must be listed in your policy. 

A written warning will be issued to drivers who incur X or more Form-and-Manner violations while in the Phase 1 probationary status. 

A copy of the written warning and record of remedial training will be placed in the driver s personnel file. Drivers who continue to submit logs with excessive violations will be placed in Phase 3 status. 

All written warning entries must include the date and time issued, reason for the warning, and the signatures of the driver, supervisor, and/or Safety Department representative.
