Triptorelin pamoate, a synthetic analog of GnRH, acts as a potent inhibitor of gonadotropin secretion. It is indicated as a palliative treatment of advanced prostate cancer and as an alternative treatment for prostate cancer when orchiectomy or estrogen administration are not indicated or are unacceptable to the patient. [Pg.711]

Triptorelin pamoate is another superagonist of GnRH, which like nafarelin acetate contains only a single amino acid substitution (D-Trp for Gly ) when compared to the natural hormone (Fig. 7.12). In the treatment of advanced prostate cancer, it is important to reduce serum testosterone levels to very low levels, which can be achieved surgically by orchiectomy. Commercial drug products containing fine-particle active agents for intramuscular, intrasynovial, or intralesional administration are relatively common. These include products for the treatment of a variety of inflammatory conditions (methypred-nisolone acetate), bacterial infections (inipenem), acromegaly (octreotide acetate), endometriosis (leuprolide acetate), prostate cancer (triptorelin pamoate), and schizophrenia (risperidone). In some cases, these products are intended to provide therapeutic blood levels of the active agent for extended periods of time (the depot effect). Fine particles that are injected other than intravenously are not subject to the same particle size constraints as those that are injected directly into the bloodstream. [Pg.82]
