Triptoreline

Injected formulation of leuptoleine and triptoreline [57773-63 ] ate used to treat metastasized cancer of the prostrate, whereas an encapsulated formulation of bromocryptin [25614-03-3] is used to inhibit milk production in women after pregnancy. The performance of encapsulated hormonal polypeptides is weU documented (47). Injectable biodegradable mictocapsules loaded with fertUity control agents have been under development for a number of years and have been carried to various stages of clinical development (48) (see Contraceptives). 

Gonadotropin releasing hormone analogs (goserelin, buserelin, leuprorelin, triptorelin) inhibit gonadotropin release and thus lower testosterone or estrogen levels. They are used to treat breast cancer and prostate cancer.

Ovarian ablation (oophorectomy) is considered by some to be the endocrine therapy of choice in premenopausal women and produces similar overall response rates as tamoxifen. Medical castration with an LHRH analog, goserelin, leuprolide, or triptorelin, is a reversible alternative to surgery.

Diffuse non-scarring alopecia occurred in a 37-year-old premenopausal woman with relapsed breast cancer 6 months after she had started to take letrozole 2.5 mg/ day and triptorelin 3.75 mg every 28 days it resolved with topical minoxidil (20).

A 32-year-old woman had psychotic symptoms of persecutory delusions, agitation, and auditory hallucinations a few days after her second injection of triptorelin (20). Her symptoms recurred after a pregnancy, suggesting that they were due to the rapid fall in estrogen in both instances. 

For in vitro fertilization the use of triptorelin 0.1 mg/ day, with early withdrawal, caused suppressed endogenous luteinizing hormone for 10-14 days after withdrawal (26). 

Injection site reactions are common with gonadorelin receptor agonists. In 119 women randomized to subcutaneous triptorelin a local reaction (redness, pain, or bruising) was present after 1 hour in 24% and persisted for 24 hours in 9.5% (41). In another study in 105 women randomized to leuprorelin acetate, moderate local reactions occurred in 24% and severe reactions in 1% (42). 

There was one case of polydactyly with no major defects (3.4%) in the children of 35 women who had conceived while using triptorelin (78). This was probably coincidental. 

There were mild increases in serum triglyceride and cholesterol concentrations in 13 hirsute women treated with triptorelin and a triphasic oral contraceptive, in a randomized comparison of triptorelin with flutamide + cypro-terone acetate (85). Altered lipid profiles have not been described before in patients receiving gonadorelin agonists and oral contraceptives.
