Topiramate

Topiramate reduces glutamate release from neurons and antagonizes acdvadon of the alpha-amino-3-hydroxy-5-methylisoxazole-4-propionic acid (AMPA) receptor, a glu-tamatergic excitatory amino acid receptor. A double-blind, placebo-con drolled dial conducted in 296 ALS pa dents over 12 months showed a faster rate of decline in arm sdrength and no effect on survival from topiramate (Cudkowicz et al., 2003). This dial had a large dropout rate and proceeded to a phase HI dial without first exploring the effects of topiramate in the SOD model. [Pg.577]

Topiramate w as recently studied in the marmoset model of PD (Silverdale et al., 2005). Overacdve AMPA receptor-mecUated dansmission may be involved in the pathogenesis of levodopa-induced dyskinesias. Topiramate significantly reduced levodopa-induced dyskinesias without affecdng the andparkinsonian acdon of levodopa. Further studies are needed to better elucidate the potendal benefits of topiramate in PD. [Pg.577]

Lamodigine is another glutamate release inhibitor that has been stucUedinneurodegeneradve diseases. In ALS, 300 mg of lamodigine per day had no effect w hen compared to placebo in a small number of padents (Ryberg et al., 2003).
