Several testosterone esters have been formulated for intramuscular injection, with different durations of action (see Table 81-3). The shorter-acting testosterone propionate, which requires dosing three times a week, has largely been replaced with testosterone cypionate or enanthate, as they can be dosed every 2, 4, or 6 weeks in most patients. However, these testosterone salts produce suprapharmacologic patterns of serum testosterone during the dosing interval, which have been linked to mood swings in some patients. 

Testosterone itself, although it is absorbed on oral administration, is ineffective because of poor bioavailability (i.e., rapid first-pass hepatic metabolism). Even the tl/2 of injected testosterone from an oil solution is only about 4-10 minutes. The several esters listed in Table 14-8—acetate, propionate, cypionate, and enanthate—on intramuscular injection of oil solutions, allows for infrequent doses (e.g., every 2 weeks) because of relatively slow absorption and subsequent hydrolysis of these pro-drugs to the parent compound—testosterone. In a practical sense, then, these esters are more active. 

Testosterone (25 to 100 mg/mL) is available in suspension, and testosterone cypionate, testosterone enanthate, and testosterone propionate are snpphed in oil.

Testosterone, when administered by mouth, is rapidly absorbed. However, it is largely converted to inactive metabolites, and only about one sixth of the dose administered is available in active form. Testosterone can be administered parenterally, but it has a more prolonged absorption time and greater activity in the propionate, enanthate, undecanoate, or cypionate ester forms. These derivatives are hydrolyzed to release free testosterone at the site of injection. Testosterone derivatives alkylated at the 17 position, eg, methyltestosterone and fluoxymesterone, are active when given by mouth. 

Current androgenic formulations for TRT largely are restricted to injectable formulations of testosterone esters, transdermal delivery formulations (scrotal or nonscrotal patches or gel), or buccal testosterone. Marketed injectable forms of testosterone esters (e.g., testosterone enanthate, propionate, or cypionate) produce undesirable fluctuations in testosterone blood levels, with supraphysiological concentrations early and subphysiological levels toward the end of the period before the next injection. These fluctuations provide an unsatisfactory benefits profile and, in some cases, undesired side effects.
