If prenatal testing for steroid disorders remains important, and screening is increasingly adopted, the GC-MS methods described here will have to be supplanted by rapid, less labor-intensive LC-MS methods. We are actively pursuing this goal.

In gas chromatography/mass spectrometry (GC/MS), the effluent from a gas chromatograph is passed into a mass spectrometer and a mass spectrum is taken every few milliseconds. Thus gas chromatography is used to separate a mixture, and mass spectrometry used to analyze it. GC/MS is a very powerful analytical technique. Since the predatory water beetles cannot biosynthesise the steroid skeleton de novo, steroidal precursors must be obtained from exogenous sources. Bacillus-strains, isolated from the foregut of the water beetle Agabus affinis, were tested for their ability to transform steroids [101]. After incubation with androst-4-en-3,17-dione two Bacillus strains produced 13 different transformation products. Hydroxylation took place at C6, C7, Cll and C14 resulting in the formation of 6fi-, 7a-, 1 la-, and 14a-hydroxyandrost-4-en-3,17-diones. After incubation with pregnenolone the two Bacillus strains produced a variety of steroids among which 7a-hydroxypregnenolone was the major product [102]. 

A screening of about 40 strains currently used for steroid hydroxylation was undertaken, using reverse-phase HPLC and UV absorption to detect and quantitate metabolite formation. Most of the strains tested were able to metabolize extensively this substrate within a 1- to 5-day period, producing in the incubation medium variable amounts of at least eight hydroxylated metabolites (80-87), essentially depending on the strain used [189]. 

There is a way to use substitution even when you are under the strictest supervision. Athletes trying to pass tests for anabolic steroids have been known to empty their bladders, and have the substituted urine injected directly into their bladders via needle. It was shown in a motion picture like Wildcats I believe. While theoretically possible, it s painful and subject to infection. It would certainly be the most senseless way to get clean urine into the testees bladder. If this must be done, catheterization should be used. 

Presently, inhaled steroids (up to the equivalent of BDP 1000 pg/d, budesonide 800 pg/d, fluticasone 500 pg/d) should be given to patients who show an objective response to either oral or inhaled steroids (s. corticosteroid reversibility testing). For those patients who experience no symptomatic relief, the currently available evidence does not support the use of ICS for alteration of the natural history of the disease. Nevertheless, corticosteroids are effective in treating acute exacerbations in COPD and taking patients of off their ICS regimen may lead to deterioration. Oral corticosteroids (e.g. 40 mg prednisolone for ten days) are recommended for exacerbations, if... 

This reagent is quite specific for corticosteroids producing blue spots on a white background. The tetrazolium spray is used in a test for related foreign steroids in fluclorolone acetonide. 

In addition, a total of 146 sheep urine and 87 chicken muscle samples from birds sold in local markets and originating from Brazil, Denmark, France, and Turkey were tested for residues of diethylstilbestrol and ethinylestradiol (41). Although some of the samples were positive to both analytes by an immunochemical screening assay, confirmatory analysis by GC-mass spectroscopy (MS) showed that none of the samples contained residues of the examined steroids.

A number of anti-inflammatory drugs have now been tested for their therapeutic efficacy in AD. For example, the steroid prednisolone, which is lipophilic, has been administered to patients with AD for up to a year but the results were disappointing. Numerous examples of the ulitity and selectivity of these sorbents are. given below. Table 2.3 lists nine steroids that were tested for their retentiveness on five different sorbents [31]. The steroid standards at 1 mg/ml were dissolved in methanol-water for the evaluation of a C,8 sorbent. For all the other sorbents, the steroids were dissolved in methylene chloride. At the polarity extremes for these steroids, cholesterol (the least polar) is retained only on C18, whereas hydrocortisone (most polar) is retained on all five of the sorbents tested. 

Immunoassays (IA) and radioimmunoassays (RIA) are widely accepted and routinely applied for clinical and environmental analyses of steroid hormones, but they have limitations in specificity and accuracy.
