Administration of penicillamine has been associated with many adverse reactions, some of which are potentially serious and even fatal. The nurse carefully evaluates any complaint or comment made by the patient and reports it to the primary health care provider. Increased skin friability may occur, which may result in easy breakdown of the skin at pressure sites, such as the hips, elbows, and shoulders. If the patient is unable to ambulate the nurse changes the patient s position and inspects pressure sites for skin breakdown every 2 hours.

Angiopathy of large (macroangiopathy) and small (microangiopathy) vessels is also the result of high blood glucose concentrations. Angiopathy results in ischemia and skin breakdown.

Yamamoto and Yamamoto (1981) studied human skin tissue and found the limit current of linearity to be about 10 pA/cm at a frequency of 10 Hz. Grimnes (1983b) studied electro-osmosis in human skin in vivo and found a strong polarity-dependent nonlinearity. The effect was stronger the lower the frequency, Figure 10.17 shows the dramatic effect with 20 V and 0.2 Hz, soon leading to skin breakdown. Nonlinearity of cardiac pacemaker CC electrodes made of noble metals and intended for use with pulses has been extensively studied (Jaron et al. 1969). 

The most important current limiting resistance of the human body is the dry skin. This may be impaired be high-field electrical breakdown, skin moisturizing, or a skin wound. Skin breakdown may occur at less than 10 V AC 50/60 Hz from electro-osmotic breakdown (Grimnes, 1983b).

The combination of weakened perineal muscles, urinary tract as well as other infections, and interruptions in peristalsis (whether neurogenic or medication induced) all contribute to a common problem among ventilator-dependent individuals—stool and urine incontinence—with its attending consequence of skin breakdown. Reduced ambulation, the inability to transfer to a toilet for evacuation, and general cognitive impairment often impair resolution of these issues. The resulting low self-esteem becomes a further barrier. 

A bowel program is essential as nothing is more disappointing for the patient than an unwelcome bowel movement after washing, dressing, and being placed in an electric wheelchair. One untimely accident can waste an entire day of rehabilitation. Bowel programs prevent skin breakdown, decrease medical complications, and improve the time spent in rehabilitation activities. 

To maintain intact skin during the establishment of the program, a basic barrier wash of oleic acid, soybean oil, and potassium hydroxide followed by a moisture barrier cream of dimethicone 3% and glycerin stearate should be used. Candida dermatitis should be treated with an antifungal product and a cortisone cream for at least 14 days. Severe dermatitis or skin breakdown on the penis may require the replacement of an indwelling catheter until healed. Urethral ulceration and labia inflammation or polyps from catheter pressure can be treated with anti-inflammatory cream twice a day, for seven days, and then tapered slowly to twice a week as a maintenance program.
