Simple checklists

While Deming s work serves as the foundation of TQM, the Baldridge award and many other quality systems, it requires translation to turn his points into the elements of a management system. His points can be used as a simple checklist, but are hard to measure against or even to interpret consistently—making it difficult to use them as a diagnostic tool. 

A series of hardware, instrument, and procedure changes were recommended and accepted to eliminate any repeat performances. One of the ways similar incidents can be reduced without resorting to more hardware or software is the religious use of simple checklists that require initials and the time each step in the start-up or shutdown process was completed. 

Although we do not advocate a strictly linear approach, it is most easily communicated through a series of steps, each comprising simple checklists that the risk assessor and manager might usefully adopt. 

Since What-If reviews are somewhat considered without direction, they are usually combined with a simple checklist to improve their efficiency.

The development requires only a limited number of expert personnel in most cases, who evaluate lists published in the literature, general publications of relevance and reports on experiences [e.g. 4, 9]. The German guideline on identification and control of exothermic chemical processes contains a simple checklist of possible deviations with negative effect on the progress of the desired process, which can be used as a basis for the development of one s own lists [8]. 

It is also possible to provide later analyses that combine the effects of several simple checklist responses, as in Parsons s (1992) thermal audit, where no single measure would exceed good practice even though the overall result would be cumulative heat stress. 

Simple checklists list the components or aspects, usually of the environment, that might be considered by the assessor, but no other assistance is provided to guide the impact identification process. 

It was revealed that most of the events due to a work planning problem where a work procedure is provided occurred during low-power states or startup operations. The reason for this can be inferred as the variable characteristics of plant configuration and dynamics of the low-power states of NPP, which may cause the identification of human error potentials and prediction of physical transition to be difficult. Therefore, the identification of human error possibilities or potentials during low-power states seems not to be an easy task to be accomplished by a list of simple checklist items, but belongs to a hard task that requires a careful investigation on the potential of human error by a concerted effort between experts in plant systems and hiunan errors, and, as necessary, may requires the use of thermal-hydraulic and reactor analysis computer codes.

In general, checklists systematically organize your work to obtain a condition of efficiency, to identify team members roles and accountability. As a consequence of checklists, error rates will decrease and patient safety will be maximally ensured. In the broadest sense, checklists are used to describe the entire surgical procedure [18] or more specifically, critical segments of the OR procedures. In an international multicenter study of3,733 controls and 3,855 patients subjected to a simple checklist in 8 hospitals, mortality dropped from 1.5 to 0.8% (p = 0.003) and in-hospital complication rate fell from 11 to 7% (p = 0.001) [19]. 

This book is not intended to deal with hazards in any depth but it may be helpful to have a simple checklist of hazards and perhaps keep adding to it as ideas develop. Please bear in mind that the subject of hazard studies is a large one and that what we cover in this book can only serve as an introduction, sufficient perhaps for you to understand their function. You should at least know what is involved in setting up a hazard study for a process or machine.
