In the United Kingdom an advisory committee was set up by the Health and Safety Executive (HSE) to review the problem and recommend procedures for the control of these sites, HSE (1976, 1979). Subsequently, a series of directives and amendments on this subject were issued by the European Economic Commission (EEC), and the EEC directives were implemented in the UK by the publication of the Control of Major Industrial Accident Hazards Regulations, 1984 (the CIMAH regulations). 

There are various types of analyses that are used for a process hazard analysis (PHA) of the equipment design and test procedures, including the effects of human error. Qualitative methods include checklists, What-If, and Hazard and Operability (HAZOP) studies. Quantitative methods include Event Trees, Fault Trees, and Failure Modes and Effect Analysis (FMEA). All of these methods require rigorous documentation and implementation to ensure that all potential safety problems are identified and the associated recommendations are addressed. The review should also consider what personal protective equipment (PPE) is needed to protect workers from injuries. 

There are two possible types of comment within an Exception Report. It may be that the client accepts an identified problem but that the recommendation cannot be implemented for various reasons. In this case, the Exception Report should describe the alternative measure to be implemented. Discussion between Road Safety Auditors and designers may help to come up with suitable alternatives. 

It is good practice to send out a feedback form with Road Safety Audits, with the objective of getting clients to tell the Road Safety Auditors how many of the safety problems are accepted, and how many of the recommendations will be implemented. From the feedback forms returned to TMS Consultancy, it is ascertained that around 60—70% of audit recommendations are implemented. 

The SaferCity Project in Gloucester in the 1990s produced Exception Reports in response to all Road Safety Audits carried out by TMS. Overall, 73% of the Road Safety Audit problems were accepted, with SaferCity either fully accepting the recommendation or implementing an alternative solution. 

It was concluded that even when a plant without any events registered during its lifetime can have hidden hazards on its conception that can occur at any time, leading to terrible accidents. Therefore is indispensable as early as possible a study to identify and correct potential hazards and operability problems to ensure a more secure installation and safer operations. Nevertheless, It is essential that the HAZOP recommendations are implemented in order to eliminate those potential hazards, improving and increasing the overall safety of this particular facility.
