The LQP is the only general optimal control problem for which there exists an analytical representation for the optimal control in closed-loop or feedback form. Once the investigation is complete, the team leaders and the managers who receive the report should provide their feedback. A representative feedback form is shown in Table 11.13. 

A template for prescriber feedback shows prescribers how their order writing could lead to harm, just as the police use special signs with embedded speed detectors to show drivers just how fast they are going, the feedback form gives prescribers immediate, personal feedback. Pharmacists make a copy of the prescriber s order, note the elements that are unsafe, attach the feedback form, and send it to the prescriber. Refer to Appendix Four. 

APPENDIX FOUR. MEDICATION SAFETY TEAM FEEDBACK FORM... 

In the tennis indusby prototypes are supplied to internal users for testing. These test matches mainly happen during work hours. In order to have reliable feedback from these product tests, embedded lead users are encouraged to put their focus on certain issues, which are critical for product success. Afterwards feedback forms are disseminated where users are supposed to give feedback. 

Some organisations use a less formal system than an Exception Report, such as a simple feedback form. The form can be used in response to all audits even when all recommendations are to be adopted. 

It is important that a copy of the Exception Report or feedback form is returned to the Road Safety Audit Team. Without some form of feedback. Road Safety Auditors are working in a vacuum and will find it difficult to improve their Road Safety Audits over time. 

While the Road Safety Auditor concentrates on road safety issues, the client or design project engineer will have to weigh up the various consequences of implementing the recommendations within the Road Safety Audit Report. Generally, the project engineer will prepare an Exception Report or at least a feedback form explaining why recommendations have been rejected. However, occasionally there will be situations where decisions are very difficult and in these cases it may be necessary to introduce a system of arbitration. 

It is good practice to send out a feedback form with Road Safety Audits, with the objective of getting clients to tell the Road Safety Auditors how many of the safety problems are accepted, and how many of the recommendations will be implemented. From the feedback forms returned to TMS Consultancy, it is ascertained that around 60—70% of audit recommendations are implemented.
