Safety instrumentation systems hardware issues

Where loss of control could lead to severe consequences, the integrity of the basic process control system and the protective safeguards must be designed, operated and maintained to a high standard. Industry standards such as ANSI/ISA-S84.01 (1996) and IEC 61508 (2000) address the issues of how to design, operate and maintain safety instrumented systems such as high temperature interlocks to achieve the necessary level of functional safety. The scope of these standards includes hardware, software, human factors and management (HSE 2000). [Pg.108]

On November 26, 1993, the USNRC issued Information Notice 93-89, "Potential Problems witii BWR Level Instrumentation Backfill Modifications," to alert licensees to potential problems that have been identified involving hardware modifications to the reactor vessel water level instrumentation system. This information involved the potential to pressurize the reference legs of the water level instrumentation if a backfill system is installed with the injection point on the instrumentation side of the manual isolation valve in the reference leg. If that valve is closed inadvertently during backfill system operation, the closure could result in a severe plant transient. At some plants, valve closure would cause all safety relief valves to open and potentially impact ECCS response. Licensees were advised to review the information for applicability to their facilities and consider actions, as appropriate, to avoid similar problems. [Pg.205]

DQ checks that the design documentation matches the requirements stated in the EDS. Layout drawings, operating and maintenance procedures, parts lists and system descriptions are included in the DQ. Both hardware and software configuration are reviewed together with the supplier s product documentation. DQ offers the opportunity to monitor the progress of project validation as well as any operational and safety issues, such as the use of radioactive instruments or access to analysis equipment for maintenance. [Pg.244]
