The first safety cases prepared for the process industries were those developed for North Sea offshore oil and gas operations following the Piper Alpha disaster that occurred in the year 1988. The Cullen report (Cullen, 1990) that was written following that accident was highly critical of offshore operating practices and recommended that a safety case approach be implemented. The Seveso incident that occurred in Italy further prompted the increased use of safety cases. Since that time the use of safety cases has spread to other industries (such as mining and railway operations) and to other nations, primarily in Europe and Australasia. (It is notable that the safety case regime approach has not been taken up for offshore oil and gas operations in the Gulf of Mexico—instead a more prescriptive approach based on industry consensus standards is used.)... [Pg.104]

The original work on wet chemical functionalization of germanium was reported as early as 1962 by Cullen et al. (researchers in RCA labs) [105]. They exposed ethyl Grignard to a chloride-terminated Ge(lll) surface and used radiotracer studies with tritiated ethyl groups to show that ethylation of the surface occurs via formation of Ge—C bonds in a one-to-one ratio with surface atoms. Because the chlorination procedure used by the authors can lead to formation of mono-, di-, and trichloride species, there may be some uncertainty as to the Ge/C ratio. However, these surfaces did demonstrate hydrophobic behavior and stability in both atmosphere and aqueous solution. [Pg.347]

The safety of the medication system is the primary concern of every hospital pharmacist. Recently published reports of the incidence, causes, and cost of injury resulting from medication errors have led many hospitals to critically evaluate their quality assurance systems (Bates et al., 1996, 1997 Classen et al., 1997 Kohn, Corrigan, and Donaldson, 1999 Leape et al., 1991, 1995 Poon et al., 2006 Stelfox, et al., 2006). Dysbindin-1 proves to be a ubiquitous neuronal protein in the CNS under optimal immunohisto-chemical conditions. Some studies have reported conservative behavior during estuarine mixing.
