Nursing homes have created units to care for noncritically ill VAIs who are free from acute medical problems. A rehabilitation nurse is often the principal provider of care, including education on self-management. The nurse, social worker, recreation therapist, and dietician work together to coordinate a program that best fits the patient. Ongoing access to an inpatient medical service is usually provided. [Pg.186]

Few physiotherapists or occupational therapists have the opportunity to work with LTMV patients and, therefore, the on-call staffs are sometimes intimidated by ventilators, airway clearance issues, and communication challenges. The PT and OT work closely together, the former helping patients to maintain muscle strength and function and the latter focusing on transfers, seating, and communication. The recreational therapist enables CAVC patients to participate in many individualized activities. Specialized equipment is always in short supply and staff support is required for community outings. [Pg.514]

In CRA with alcohol-dependent patients, reinforcement of disulfiram compliance is one of the primary components of treatment, such that use of alcohol loses its reinforcing aspects and, in fact, becomes aversive. Furthermore, reinforcement from other sources is increased. Positive reinforcement for not drinking comes in the form of scheduhng other recreational activities and reorganizing daily life by breaking down pracrical barriers. For example, the therapist may assist the patient in obtaining a telephone, a place to hve, or transportation to treatment. [Pg.346]

Values matter. In our personal lives—our relationships with family and friends, and in our choice of work and recreation—I believe in individual liberty. People should not accept emotional or physical bullying or coercion in their personal or professional lives. In the political realm, the problem of individual freedom obviously becomes more complicated, but in our personal lives, it can be straightforward. In our personal lives, we should respect each other s freedom. As therapists, we respect the freedom of our patients, and we encourage them to respect the freedom of others (see my discussions of liberty, love, and oppression from an individual and societal perspective in Breggin 1988, 1988-1989, and 1992a). [Pg.453]

The therapist can also help the family by advising them of educational sources and available support groups, such as the local chapter of the Alzheimer s Disease Association, social service and home health agencies, assistance with home behavioral management, Meals-on-Wheels, transportation and recreational programs, and respite care. [Pg.142]

Keep in mind that medications and other drugs are a very common cause of emotional symptoms—this includes prescription, nonprescription, and other recreational drugs and alcohol. Obtaining collateral history from family members may be helpful. Also conducting the "bag test"—having the person bring in all (this cannot be stressed too much) medications he or she is taking for the therapist to see and make a record of. [Pg.57]
