Recombinant growth hormon

Recombinant growth hormone therapy is the main pharmacologic treatment for growth hormone deficiency in both children and adults. 

Although comparative trials have not been conducted, recombinant growth hormone products appear to have similar efficacy for treating growth hormone deficiency.

Despite the lack of controlled clinical trials supporting its use, many clinicians supplement PN therapy with glutamine and recombinant growth hormone in an attempt to increase intestinal absorption and facilitate transition off PN. Recombinant growth hormone is a costly product with adverse effects, but PN is more costly and also has a serious adverse-effect pro-file. Larger controlled clinical trials are needed to determine the benefit of glutamine and rhGH in this patient population. 

In a project implemented at Nil, ovine growth hormone (oGH) was expressed as inclusion bodies using a pQE expression vector. The expressed protein was 10 -15% of the total cellular proteins of E. coli. The recombinant ovine growth hormone was purified from inclusion bodies and refolded into its native conformation. The refolded recombinant oGH was found to be immunoreactive with RIA/RRA characterized by SDS-PAGE, Western blot, N-terminal amino acid sequence, CD, fluorescence spectra, RIA, and RRA. High cell density cultivation was standardized and found to be enhanced to 100 times. In 16 h, 3.2 g H of oGH was produced which is claimed to be the highest level of any recombinant growth hormone reported in E. coli [57]. 

Human growth hormone has also been produced in E. coli and is used to treat children with growth hormone deficiencies. Before production of recombinant growth hormone, growth hormone isolated from cadaver pituitary tissue was used, which was in short supply. 

Given the possible relationship between decreases in protein synthesis and ventilator-associated respiratory muscle dysfunction, it would seem plausible that administration of anabolic factors, such as growth hormone, might be beneficial to ventilated patients. Unfortunately, when growth hormone was administered to patients requiring PMV, duration of mechanical ventilation was not decreased nor was muscle strength increased (80). The report that recombinant growth hormone can increase mortality of critically ill patients is a matter of concern (81).
