There are two important safety measures that can be taken at very low cost. The first is to equip shelves with guard rails to keep reagent bottles from falling. The height of such rails should be adapted to the size of the containers. This is a requirement in California. The second is to fasten tall objects to the wall, a simple and inexpensive procedure. On August 14, 2002, a 1-in chlorine transfer hose (CTH) used in a rail-car offloading operation at DPC Enterprises in Festus, Missouri, catastrophically ruptured and initiated a sequence of events that led to the release of 48,000 lb of chlorine into neighboring areas. The material of construction of the ruptured hose was incorrect. The distributor fabricated bulk CTH with Schedule 80 Monel 400 end fittings and a high-density polyethylene spiral guard. Three hoses were shipped directly to the Festus facility from the distributor two were put into service on June 15, 2002. The hose involved in the incident failed after 59 days in service. [Pg.6]

The Dept of Transportation has combined the shipping regulations which used to be separately administered by the US Coast Guard (Title 46, CFR Part 146) and the Federal Aviation Regulations (Title 14, CFR Part 103) (Ref 46). The new regulation provides for a uniform vehicle placarding and documentation system. They replace rail express criteria with a hazard assessment for aircraft operation, and uniform criteria for all shipping modes have been established. In June of 2005 and June 2006, AC174 was partially modified. It is now based on a newly introduced ASTM D 7032-05. Besides, as it was stated above, ASTM D 7032 says (Section 6.2.3) that the top rail of the tested guard system shall resist a uniform load (actually, a quarter-point loading) of 125 Ib/ft, applied vertically and horizontally. The new AC174, however, states that when compliance with SBC code requirements is desired, the stated uniform load in Section 6.2.3 of ASTM D 7032 shall be increased to 235 Ib/ft (applied in an outward direction at an angle of 58° from horizontal, plus any adjustments [temperature and moisture, UV light, and freeze-thaw effects]). [Pg.307]
