Methanol is metabolized to formaldehyde and formic acid, which injure the retinal cells and optic nerves, and lead to severe acidosis. Treatment delay increases morbidity. Thus, early recognition and management are crucial. Clinical features emerge up to 36 hours after ingestion. Nausea, vomiting, abdominal pain, headache, dizziness, paraesthesia, blurred vision, and diminished visual activity may occur, and coma supervenes. Dilated, unreactive, pupils predict permanent blindness. 

In overdose, the antihistamines cause convulsions, hallucinations, excitement, ataxia, incoordination, and athetosis. On exam patients may exhibit fixed, dilated pupils with a flushed face, sinus tachycardia, urinary retention, dry mouth, and fever. At high doses the patient can become comatose, which is often followed by cardiorespiratory collapse and death within 2 to 18 hours (Babe and Serafin, 1996). Treatment of overdose is mainly supportive, with efforts to manage the anti-colinergic effects. 

An 85-year-old woman became gradually toxic (serum lithium 2.9 mmol/1) in a nursing home (168). Despite only conservative management, there was slow but complete resolution of severe neurological symptoms, including coma, fixed pupils, and Cheyne-Stokes respiration.

Because Adie s syndrome is a benign disorder, the most important aspect in patient management is reassurance. The associated accommodative paresis tends to recover during the first several years, and any visual impairment thus improves. The patient should be advised that the second eye may become involved but that the other changes associated with the syndrome (decreased light reaction and diminished deep tendon reflexes) do not represent significant functional impairments. For many patients the chief concern is the cosmetic appearance of the unequal pupils. Most patients can be reassured that, with time, this should become less noticeable.
