Thorough and effective analyses of workplace incidents are critical components of a comprehensive safety management system. Yet, many incident analysis processes (i.e., accident investigations) fall short. They frequently fail to identify and resolve the real root causes of injuries, process incidents and near misses. Because the true root causes of incidents are within the system, the system must change to prevent the incident from happening again. 

However, a thorough incident investigation into such an apparently simple accident could lead to the discovery of significant and subtle deficiencies in the overall management program that could, in turn, lead to ways of improving process safety. 

Central to the safety case is the hazard control and risk manag ent process, ensuring that risks are well managed. ALARP is used to determine how far you should go to control the hazard. The safety case process is very thorough and highly labor intensive and, if you are not careful, could become overly bureaucratic. It is just a snapshot in time of the safety of the system, so that means that it must be maintained to still be relevant. This is where bureauCTacy can take over if not careful. 

EMPLOYEE SAFETY AS WELL AS A CLEAN ENVIRONMENT WILL REQUIRE MORE SOPHISTICATED CONTROL AND THOROUGH DOCUMENTATION THAT WILL CERTAINLY AFFECT HOME OFFICE HOURS. PROJECT MANAGEMENT HOURS AND CONTROL HOURS AS WELL AS PROCESS AND INSTRUMENTATION WILL BE AFFECTED. AN INCREASE OF 15-20% WOULD BE JUSTIFIED.
