Pre Start-up Safety Review

In one site, the project team installing a DCS (Distributed Control System) carefully developed and tested techniques to make the displays clear for red-green color blind personnel (see discussion in 6.4). The displays were effective and were applauded by the operators. However, in subsequent DCS installation projects at the same site, different project teams made no provision to make displays visible to red-green color blind personnel. This inherently less safe condition was found during a design review at one unit and at the pre-start-up safety review for another unit. 

Pre-start-up Safety Review (PSSR) approval of the SIS establishes the point where the SIS design and construction is considered complete. All documentation should be formally updated to as-built status, incorporating any modifications made since the last formal drawing or document revision. Once the PSSR has approved the SIS for process unit start-up, formal management of change procedures should be followed to address proposed modification to the SIS or its associated documentation. Any deviation from the approved design basis should be reviewed and approved by appropriate parties prior to change implementation. 

Much like a PrHA, the pre-start-up safety review is conducted by a team of experienced and knowledgeable personnel that examine the physical facility, associated documentation to include procedures, and proposed workers. No live processing is accomplished prior to the Pre-start-up safety review. 

A tracking system should also be in place to follow the findings and recommendations until a resolution has been achieved. The process should not be operated with unresolved pre-start-up safety review findings, issues, or recommendations. 

The pre-start-up safety review should be in the level of detail to assure safe operation and there should be a pre-start-up safety review plan of action should be developed and approved by management.

The ISA84 committee created ANSI/ISA-84.01-1996 to supplement OSHA 1910.119 in the areas related to the implementation of instrumentation and controls necessary for safe operation. Rather than repeating OSHA 1910.119 mandates, the standard references OSHA 1910.119 for some key program elements. Specifically, ISA-84.00.01-1996 does not provide specific requirements for safety management, hazard and risk analysis, pre-start-up safety review, or training. ISA-84.00.01-2004 provides requirements for each of these areas.

A safety review is mandated for new facilities and significantly modified work sites to confirm that the construction and equipment of a prcx ess are in accordance with design specifications to assure that adequate safety, operating, maintenance, and emergency procedures are in place and to assure pr(x ess operator training has been completed. Also, for new facilities, the PHA must be performed and recommendations resolved and implemented before start up. Modified facilities must meet management of change requirement.
