Pre-Start-up Safety Review PSSR

In order to carry out PSSR, the team will require a suitable checklist. Relevant process safety items must be addressed in the inspections by having key topics defined fully within the PSSR checklist (CCPS, 2004). A typical checklist to be completed by the PSSR team includes the following but not limited to. 

PSSR is essential to ensure that the new/modified facility will perform as envisioned. The larger and more complex the project, the greater is the need to perform PSSR with care and in detail. Large revamp projects tend to fall behind schedule and, near the end, there is usually pressure to cut comers to speed up the schedule. This is also the point in time where the potential for mistakes may be greatly increased. Under these circumstances, it is even more important to step back and conduct a thorough PSSR.

Pre-start-up Safety Review (PSSR) approval of the SIS establishes the point where the SIS design and construction is considered complete. All documentation should be formally updated to as-built status, incorporating any modifications made since the last formal drawing or document revision. Once the PSSR has approved the SIS for process unit start-up, formal management of change procedures should be followed to address proposed modification to the SIS or its associated documentation. Any deviation from the approved design basis should be reviewed and approved by appropriate parties prior to change implementation.

The Project Manager should maintain and issue a action plan summary until all items are closed out. The HAZOP or What-If Addendum report is usually prepared from the action plan summaries. Items which are not closed out prior to the facility or project start-up should be addressed as part of the Pre-Startup-Safety-Review (PSSR). A copy of the action plant should be made available to operating, maintenance and other employees whose work assignments are in the process and who may be affected by the recommendations or actions. 

The second-level supervisor or department head ensures that PSSR requirements have been addressed and documented in the PSSR Form. He may elect to secure additional approvals. His authorization signature (line 9) grants approval to start up the new or changed facilities. If any of the pre-startup requirements have not been met satisfactorily, he notifies the supervisor of the affected process unit to correct the deficiencies and repeat the Pre-Startup Safety Review. (Note Authorization to proceed with the change, completion of the PSSR, and approval for startup may occur at the same time for changes of a minor nature.)...
