Uterine atonia, postpartum bleeding, migraine, orthostatic circulatory disturbances, senile cerebral insufficiency, hypertension, hyperprolactinemia, acromegaly, and Parkinsonism [36]. New therapeutic uses have recently emerged, such as, e.g., against schizophrenia, applications based on its newly discovered antibacterial and cytostatic effects, and immunomodulatory and hypolipemic activity [37-39]. 

Oxytocin may be given by intravenous infusion (e.g., labor induction), intramuscular injection (e.g., control of postpartum bleeding), or as a nasal spray (e.g., to promote milk ejection). 

Carboprost has been used successfully to control postpartum bleeding that was secondary to loss of uterine tone and where the myometrium was unresponsive to oxytocin, ergonovine, or methylergonovine. Given intramuscularly, carboprost causes an almost immediate and sustained uterine contraction. Clinical experience has shown that the use of this agent has saved many women from operative interventions (including hysterectomy) to control postpartum hemorrhage.

Ox ocin is a nonapeptide which is structurally related to vasopressin. It stimulates rhythmic uterine contractions and is widely used by intravenous infusion of a diluted solution to induce labour and to treat postpartum bleeding. In large doses, it may cause relaxation of vascular smooth muscle causing hypotension in patients with cardiac disease or who are dehydrated. It has water-retaining properties and when given for prolonged periods to patients whose intake is electrolyte-free it causes overhydration and hyponatraemia. This may result in convulsions in the newborn with the risk of cerebral damage.

Oxytocin is administered intravenously for initiation and augmentation of labor. It also can be administered intramuscularly for control of postpartum bleeding. Oxytocin is not bound to plasma proteins and is eliminated by the kidneys and liver, with a circulating half-life of 5 minutes.

In an in vitro experiment using uterine muscle strips from 10 consenting parturients undergoing cesarean section, therapeutic concentrations of propofol had no effect on isometric tension developed during contraction of the muscle (68). However, higher than therapeutic concentrations did reduce the peak muscle tension that developed. These results confirm that propofol is free of this adverse effect, which is a known cause of postpartum bleeding after the use of volatile anesthetic drugs. 

This drug is administered by parenteral route for induction of labor and control of postpartum bleeding, or by the intranasal route for induction of milk let-down.

Early references note, and contemporary herbalists maintain, that extracts made from the fresh herb are more active than those from the dried herb (Felter and Lloyd 1898, 1901). While this does not relate to the safety of the extract itself, one of the primary indications of shepherd s purse is to stop postpartum bleeding, a setting in which the use of a nonefficacious product may present a safety concern.
