Pentothal - Thiopental

The carbonyl oxygen at C-2 of pentobarbital is replaced by sulfur in Pentothal (thiopental).

The ultra-short-acting barbiturates include methohexital sodium (Brevi-tal) and thiopental sodium (Pentothal). These agents are used as anesthetics and are administered intravenously. Barbiturates with short-to-intermediate duration of action are used for their sedative-hypnotic effect in the treatment of anxiety. These medications include amobarbital (Amytal), butabarbital (Butisol), sodium pentobarbital (Nembutal), and secobarbital (Seconal). 

In general, barbiturates—thiopental in particular—change into soluble form on treatment with bases. Therefore, thiopental often appears in the market under the name sodium thiopental. In this case, the formation of a salt occurs due to the sulfur atom in an enethi-olate form. The most common synonyms for thiopental are pentothal, trapanal, farmotal, intraval, and others. 

Methohexital is also an extremely short-lasting barbitnrate, and it is nsed in same circumstances as is thiopental. It has a slightly shorter active time than thiopental however, this difference is insignificant in clinical situations. Synonyms of methohexital are pentothal, intraval, farmotal, ravonal, and others.

Brief History. B.W., a 75-year-old woman, fell at home and experienced a sudden sharp pain in her left hip. She was unable to walk and was taken to a nearby hospital where x-ray examination showed an impacted fracture of the left hip. The patient was alert and oriented at the time of admission. She had a history of arteriosclerotic cardiovascular disease and diabetes mellitus, but her medical condition was stable. The patient was relatively obese, and a considerable amount of osteoarthritis was present in both hips. Two days after admission, a total hip arthroplasty was performed under general anesthesia. Meperidine (Demerol) was given intramuscularly as a preoperative sedative. General anesthesia was induced by intravenous administration of thiopental (Pentothal) and sustained by inhalation of halothane (Fluothane). The surgery was completed successfully, and physical therapy was initiated at the patient s bedside on the subsequent day.

The ultrashort-acting barbiturates methohexital (Bre-vital), thiamylal (Surital), and thiopental (Pentothal) are administered as an anesthetic before surgery. The barbiturate is injected into the patient s vein to induce anesthesia. The patient then receives a longer-acting anesthetic like nitrous oxide. The drugs are used in hospital settings and are not subject to abuse, according to the DEA. 

After the September 11, 2001, terrorist attacks in the United States, federal investigators discussed the use of truth serum to obtain information from jailed terrorism suspects. The serum is the barbiturate thiopental, which is also known as pentothal sodium. The use of this drug to make a reluctant person talk has been portrayed in movies, books, and on television. 

In large doses, injected intravenously, it would bum and hurt horribly, because it s a salt and because it instantly throws off the chemical balance of the blood with which it comes into contact. It makes all muscles lock up in extreme contraction that would hurt unbearably. It wouldn t get to all muscles when a prisoner is being killed with it, however. Since the heart is a muscle and it pumps the blood -- the minute that massive dose of potassium salt hits the heart, one would be history and that would be as far as it would travel. "In lethal injection, three chemicals are used to kill. First, sodium pentothal (its trade name) or thiopental sodium (its chemical name). Then one minute later they inject pavulon. One minute later the potassium chloride. Pentothal is a short acting barbituric acid (barbiturate used in anaesthesia) and is commonly called "truth serum" as it s used in narcoanalysis. It knocks one out. It s a hypnotic. Pavulon is a curare derivative which locks up the lungs so one can t breathe.

The compound s onset of action is about equal to the time required for it to travel to the brain from the site of administration. Consciousness is regained within 30 minutes.
