Organization, hazard review team

Diversity is recognized as a useful approach to reduce the number of defects. The team that conducts the process hazards analysis does not implement the safety interlocks but provides the specifications for the safety interlocks to another organization for implementation. This organization reviews the specifications for each safety interlock, seeking clarifications as necessary from the process hazards analysis team and bringing any perceived deficiencies to the attention of the process hazards analysis team. 

If a proposed change falls outside of plant specifications, codes of practice, or is relatively new technology, then a chemical process safety review team evaluation may be in order. Different organizations provide various names to process safely review teams, such as Safe Operations Committee, Hazards Review Committee, Technical Support Team, Facility and Operations Change Review Committee, or Chemical Process Safety Review Team. Supported by the PSMT, the line organization develops a structured approach for performing process hazard analyses on a repetitive basis. One effective way to do this is to organize knowledgeable facility personnel into ad hoc Process Safety Review (PSR) teams. These teams, supplemented by outside specialists as required, are responsible for studying all processes, identifying all potential hazards, and recommending appropriate corrective or control measures. 

As identified in a safety review (e.g., process hazard analysis [PHA], What-lf Analysis, Hazard and Operability Study [HAZOP]), a defined part (section or subsystem or item of equipment) of a process that has a design intention that is specific and distinct from the design intention of other process parts, which allows the study team to analyze the specific equipment or system in an organized fashion. 

Routine development and use of job hazard analyses as well as education of employees about each job s hazards and associated risk. The process should include involvement or comprehensive communication with the leadership team so that this understanding is present throughout the organization. The SEMP also describes the structure of the project teams and outlines the project deliverables, decision database, specifications, and baselines. The SEMP may be tailored to fit the size and complexity of the project. It may be generated so that the template may be reused on future projects in the organization. Appendices are used to capture milestones, technical review schedules, and the roadmap for resolution of problems and identified hazards throughout the life cycle.
