To distinguish between a review and an audit, some definitions will be provided. A review is a critical examination or evaluation of any operation, procedure, condition, event, or equipment item. Reviews can take many forms and be identified as project reviews, design reviews, safety reviews, pre-start-up reviews, and so on. The following discussion of the review process will deal with project reviews associated with capital projects and focus on the area of process safety. [Pg.2283]

It is also helpful to include conditions in a timeline however, it is important to distinguish between events and conditions. Conditions tend to be passive items, such as the pump was running, the pipe was corroded, or the operators were not trained on the draining procedure. Condition statements are identified by the words was or were. Events by contrast are active, such as the pump started up or the pipe failed. Both events and conditions can be facts. Industrial scale polymer forming operations are usually based on the combination of various types of individual processes. Therefore in the computer-aided design of these operations a section-by-section approach can be adopted, in which each section of a larger process is modelled separately. An important requirement in this approach is the imposition of realistic boundary conditions at the limits of the sub-sections of a complicated process. The division of a complex operation into simpler sections should therefore be based on a systematic procedure that can provide the necessary boundary conditions at the limits of its sub-processes. In general, the imposition of boundary eonditions is a part of the assembly process. A simple procedure for this is to assign a eode of say 0 for an unknown degree of freedom and 1 to those that are specified as the boundary conditions. The ease of fault detection, isolation, and repair are all influenced by system design and are principal factors contributing to maintainabiUty.
