Native pancreas

For the kinetic resolution of racemic a-acetoxyamide 6 several native enzymes were used (Scheme 5.5). The native Upases from Pseudomonas cepacia (PCL) and porcine pancreas (PPL) showed the highesL although stiU unsatisfactory, enantios-electivity ( = 5.1 and 3.5, respectively). Upon immobilization into a solgel matrix, the enantioselectivity of PCL was improved significantly to 30.5. The covalent immobilization on Eupergit increased the enantioselectivity even more (34.0) [23]. 

Most kidney transplant procedures are heterotopic, where the kidney is implanted above the pelvic bone and attached to the patient s iliac artery and vein. The ureter of the transplant kidney is attached directly to the recipient s bladder or native ureter. The native kidneys usually are not removed, and data have shown that under most circumstances, removal of the native kidneys does not influence patient survival and allograft survival. Special circumstances, however, such as renal cell carcinoma and PCKD, may necessitate native kidney removal.1,3 There were 16,477 (9914 deceased donors, 6563 living donors) kidney transplants and 903 simultaneous kidney-pancreas procedures done in 2005.3... 

On the commercial front, an artificial liver system has reached advanced clinical trial stage. Based on pig hep-atocytes immobilized in a hollow-fiber membrane module, this system provides temporary life support until a liver from a human donor is available for transplantation (Fig. 50). Also under development is an artificial pancreas intended as a permanent replacement of the native organ (Fig. 51).

Banting, Sir Frederick. (1891-1941). A native of Ontario, Canada, Banting did his most important work in endocrinology. His brilliant research culminated in the preparation of the antidiabetic hormone that he called insulin, derived from the isles of Langerhans in the pancreas. He received the Nobel Prize in medicine for this work together with MacLeod of the University of Toronto. In 1930, the Banting Institute was founded in Toronto. He was killed in an airplane crash. 

Native HPL is a glycosylated 50 kDa protein which is synthesized by acinar ceUs of the pancreas (Fig. 10.2). In the cell, this hpase is located with many other enzymes into the zymogen granules, and is then secreted through the pancreatic duct into the intestinal lumen together with the bile. Secretion of HPL can be stimulated by intestinal hormones such as CCK-PZ and secretin (Zieve et al., 1966a,b). Insulin, Inutral Optisulin long. Polypeptide hormone produced in the beta cells of the islets of Laager-bans situated in the pancreas of all vertebrates. Synthesized in vivo via a single chain polypeptide precursor, proinsulin, native conformation has been established. Secreted directly into the bloodstream where it regulates carbohydrate metabolism, influences the synthesis of protein and of RNA, and the formation and storage of neutral lipids. Insulin is synthesized as preproinsulin that is proteolytically processed in the P cells of the islets of Lang-erhans in the pancreas, to give proinsulin. After synthesis and folding, a section of the molecule (the C peptide) is excised, leaving the A and B peptides connected via disulfide bonds. Thus, native insulin, lacking the C peptide, lacks some of the information necessary to direct the folding process.

Rational protein engineering has been successfully used in a variety of problems of interest in biomedical engineering. One of the first, and perhaps the most widely used examples can be found with the protein engineering of human insulin for the treatment of diabetes [ 3 ]. Native insulin has evolved to form dimers and hexamers, so that it can be produced and stockpiled in the pancreas before it is needed for release in the body. When purified insulin is injected subcutaneously following a meal as a treatment for diabetes, only the active monomer form is desired, and thus the formation of dimers and hexamers can slow absorption. This has been addressed using site-directed mutagenesis to introduce repulsive charges and steric hindrances at the dimer interface, in order to reduce the tendency of human insulin to self-assemble. This work has led to insulin monomers that have an increased rate of absorption, and thus a produce a preferable postprandial plasma concentration profile [4].
